Get to know the person so you understand their perspective and reality (rather than the one that you're assuming or projecting onto them). Ask them questions like "Where are you right now?" "What are you good at?" "What are you bad at?" "What are your obstacles?" "What are your goals?" Once you understand how the person operates, you can help them do anything since you can clearly see the puzzle pieces you have to work with.
