The symptoms associated with benign prostatic hyperplasia (BPH) such as urinary frequency, nocturia, weak stream, hesitancy and incomplete emptying are related to two components, anatomical (static) and functional (dynamic). The static component is related to the prostate size. But prostate size alone does not correlate with symptom severity. The dynamic component is a function of the smooth muscle tone in the prostate and its capsule, the bladder neck and the bladder base as well as the prostatic urethra.

The smooth muscle tone is regulated by alpha-adrenergic receptors. Alfuzosin is an orally active quinazoline derivative, peripherally acting antagonist, exhibits selectivity for postsynaptic alpha-1 adrenergic receptors in the lower urinary tract. Blockade of these adrenoceptors can cause smooth muscle in the bladder neck and prostate to relax, resulting in an improvement in urine flow and a reduction in symptoms of BPH.

কাজ

ইউরিটেন পুরুষদের বিনাইন প্রােস্টেটিক হাইপারপ্লাসিয়া (বি,পি,এইচ)'র কার্যকর লক্ষণসমূহের জন্য নির্দেশিত। এটা বি, পি, এইচজনিত রােগীর মূত্র ত্যাগ এর অক্ষমতাজনিত তীব্র সমস্যার স্বল্প মেয়াদী চিকিৎসায়ও ক্যাথেটারাইজেশন সহকারে ব্যবহৃত হয়। এছাড়াও মহিলাদের মূত্র ত্যাগ এর অক্ষমতাজনিত সমস্যার ক্ষেত্রেও এটি ব্যবহার করা যেতে পারে।

মাত্রা ও সেবনবিধি

• বি পি এইচ : ইউরিটেন ১০ মি.গ্রা. এর একটি ট্যাবলেট প্রতিদিন একই বেলায় খাবারের পরপর।
• মূত্র ধারনজনিত তীব্র সমস্যা : ইউরিটেন ১০ মি.গ্রা. এর একটি ট্যাবলেট প্রতিদিন একই বেলায় খাবারের পরপর।
• ক্যাথেটারাইজেশনের প্রথম দিন থেকে ব্যবহার শুরু করতে হবে।
• চিকিৎসা ৩-৪ দিন চলবে (২-৩ দিন ক্যাথেটারাইজেশনের সময় এবং ১ দিন ক্যাথেটারাইজেশনের পর)।

Alfuzosin Hydrochloride tablet should be swallowed whole.

পার্শ্ব প্রতিক্রিয়া

আলফুজোসিন সাধারনত সুসহনীয়, খুব কম ক্ষেত্রে যেসব পার্শ্ব প্রতিক্রিয়া দেখা যায় সেগুলাে হল : মাথাঘােরা, মাথাব্যাথা, বিষন্নতা, দূর্বলতা, অস্থিরতা, অনুভূতি, হৃদস্পন্দন বেড়ে যাওয়া, বুক ধরফড় করা, বমিবমি ভাব, পেট ব্যথা, ডায়রিয়া, শুল্ক মুখ ইত্যাদি।

সতর্কতা

The administration of general anesthetics to patients receiving Alfuzosin could cause profound hypotension. It is recommended that the tablets be withdrawn 24 hours before surgery. If symptoms of angina pectoris start or get worse, taking Alfuzosin should be stopped.

ঔষধের মিথষ্ক্রিয়া

Alfuzosin may interact with other alpha-blockers, Atenolol, Cimetidine, Diltiazem, Ketoconazole and Ritonavir.

গর্ভাবস্থায় ও স্তন্যদানকালে ব্যবহার

গর্ভাবস্থায় ও স্তন্যদানকালে আলফুজোসিন ব্যবহারের জন্য নির্দেশিত নয়।

মাত্রাধিক্যতা

আলফুজোসিনের মাত্রাধিক্যের ফলে নিম্ন রক্তচাপ হতে পারে। এ ক্ষেত্রে রােগীকে হাসপাতলে নিয়ে যেতে হবে; সােজা করে শুইয়ে দিতে হবে এবং নিম্ন রক্তচাপের প্রচলিত চিকিৎসা শুরু করতে হবে।

প্রতিলক্ষণ

আলফুজোসিনগ্রহনকারী রােগীকে সাধারনত চেতনানাশক দেয়া হলে তা অতিমাত্রায় নিম্নরক্তচাপতৈরী করতে পারে। সুতরাং শল্য চিকিৎসায় ২৪ ঘন্টা পূর্বেই আলফুজোসিন ট্যাবলেট বন্ধ করা উচিৎ। এনজিনা পেটোরিসের লক্ষণ শুরু হলে অথবা অবস্থার অবনতি হলে আলফুজোসিন গ্রহন বন্ধ করা উচিত। অতিসংবেদনশীলতা, অবস্থানপরিবর্তনজনিত নিম্নরক্তচাপ, মধ্যম থেকে তীব্র যকৃতের সমস্যা থাকলে এবং অন্যান্য আলফা প্রতিবন্ধকের সাথে আলফুজোসিন প্রতিনির্দেশিত। এই ওষুধ নারী এবং শিশুদের (১৮ বছরের নিচে) ক্ষেত্রে নির্দেশিত নয়।

বিশেষ সতর্কতা

Efficacy of Alfuzosin Hydrochloride has not been demonstrated in children aged 2 to 16 years. Therefore Alfuzosin Hydrochloride is not indicated for use in the paediatric population.

অন্যান্য ওষুধের সাথে মিথস্ক্রিয়া

আলফুজোসিন অন্যান্য আলফা-প্রতিবন্ধক, এটেননালােল, সিমেটিডিন, ডিলটিয়াজেম, কিটোকোনাজোল এবং রিটোনাভিরের সাথে প্রতিক্রিয়া করতে পারে।
