This formulation contains a fixed ratio of 1:6 parts of Artemether and Lumefantrine. The site of antiparasitic action of both components is the food vacuole of the malarial parasite, where they are thought to interfere with the conversion of haem, a toxic intermediate produced during haemoglobin breakdown, to the non-toxic haemozoin (malaria pigment). Lumefantrine is thought to interfere with the polymerisation process, while Artemether generates reactive metabolites as a result of the interaction between its peroxide bridge and haem iron. Both Artemether and Lumefantrine have a secondary action involving inhibition of nucleic acid and protein synthesis within the malarial parasite.

Anti-malarial drugsমাত্রা ও সেবনবিধি
- ম্যালেরিয়া রােগীদের সাধারণত খাদ্য গ্রহনে অনীহা দেখা যায়।
- তাই লুমারটেম এর সাথে সাথে রােগীদের চর্বিজাত বা তরল (যেমন-দুধ) খাবার খাওয়ানাে উচিত।
- ওষুধ খাবার এক ঘন্টার মধ্যে বমি হলে ওষুধটি আবার খাওয়াতে হবে।
- লুমারটেম এর কাঙ্খিত ফল পেতে হলে তিনদিনে মােট ৬ বার খেতে হবে।
- ৩৫ কেজির উর্ধ্বে শিশু এবং প্রাপ্ত বয়স্কদের জন্য: প্রাথমিক রােগ নির্ণয়ের সময় ৪টি ট্যাবলেট তারপর আট ঘন্টা পর আরও ৪টি ট্যাবলেট খাওয়াতে হবে। অতঃপর পরবর্তী দুইদিন প্রতিদিন সকালে ও সন্ধ্যায় ৪টি করে ট্যাবলেট খাওয়াতে হবে (মােট ২৪টি ট্যাবলেট)।
- ৫-১৫ কেজি ওজনের শিশুদের ক্ষেত্রে: প্রাথমিক রােগ নির্ণয়ের সময় ১টি ট্যাবলেট তারপর আট ঘন্টা পর আরও ১টি ট্যাবলেট খাওয়াতে হবে। অতঃপর পরবর্তী দুইদিন প্রতিদিন সকাল ও সন্ধ্যায় ১টি করে ট্যাবলেট খাওয়াতে হবে (মােট ৬টি ট্যাবলেট) ১৫-২৫ কেজি ওজনের শিশুদের ক্ষেত্রে।
- প্রাথমিক রােগ নির্ণয়ের সময় ২টি ট্যাবলেট তারপর আটঘন্টা পর আরও ২টি ট্যাবলেট খাওয়াতে হবে। অতঃপর পরবর্তী দুইদিন প্রতিদিন সকালে ও সন্ধ্যায়।
- ২টি করে ট্যাবলেট খাওয়াতে হবে। (মােট ১২টি ট্যাবলেট)।
- ২৫-৩৫ কেজি ওজনের শিশুদের ক্ষেত্রে: প্রাথমিক রােগ নির্ণয়ের সময় ৩টি ট্যাবলেট তারপর আটঘন্টা পর আরও তিনটি ট্যাবলেট খাওয়াতে হবে। অতঃপর পরবর্তী দুইদিন প্রতিদিন সকাল ও সন্ধ্যায় তিনটি করে ট্যাবলেট খাওয়াতে হবে (মােট ১৮টি ট্যাবলেট)।
পার্শ্ব প্রতিক্রিয়া
লুমারটেম সুসহনীয় এবং পাশ প্রতিক্রিয়ার মাত্রা সাধরণত স্বল্প থেকে মাঝারী ধরনের হতে পারে। বিশেষ সতর্কতা : লুমারটেম ম্যালেরিয়া প্রতিরােধক হিসাবে ব্যবহার করা যাবে না। তাছাড়া সেরিব্রাল ম্যালেরিয়ার ক্ষেত্রেও এটি ব্যবহার করা উচিত নয়।
সতর্কতা
সংবেদনশীলতার ক্ষেত্রে; তীব্র ম্যালেরিয়ায় যাদের ক্ষেত্রে ইলেকট্রোলাইট ভারসাম্যহীনতা দেখা যায়।
ঔষধের মিথষ্ক্রিয়া
Dose adjustment of this combinationis considered unnecessary when administered in association with ketoconazole. The likelihood of interactions with other drugs is minimal in view of its short duration of administration and wide therapeutic index. From study it was found that, the risk of QTc-prolongation associated with IV quinine was enhanced by prior administration of this combination.
গর্ভাবস্থায় ও স্তন্যদানকালে ব্যবহার
গর্ভবস্থায় প্রথম ৩ মাসে এটি নির্দেশিত নয়। স্তন্যদানকারী মায়েদের ক্ষেত্রে এটি ব্যবহার করা যাবে না। তবে কেবল মাত্র যদি সুফল ঝুঁকির তুলনায় বেশী হয় তখনই ওষুধ গ্রহণের ২৮ দিনের মধ্যে স্তন্যদান করা যাবে।
মাত্রাধিক্যতা
In cases of suspected overdosage, symptomatic and supportive therapy should be given as appropriate. ECG and electrolytes (e.g. potassium) should be monitored.
প্রতিলক্ষণ
Hypersensitivity to any of the ingredients or excipients; Patients with severe malaria according to WHO definition; First trimester of pregnancy; Patients with a family history of congenital prolongation of the QTc interval or sudden death or with any other clinical condition known to prolong the QTc interval such as patients with a history of symptomatic cardiac arrythmias with clinically relevant bradycardia or with severe cardiac disease; Patients with known disturbance of electrolyte balance e.g. hypokalaemia or hypomagnesemia; Patients taking any drug which is metabolized by the cytochrome enzyme CYP2D6 (e.g. flecainide, metoprolol, imipramine, amitriptyline, clomipramine).
