Gentamicin is the most important of the aminoglycosides and is widely used for the treatment of serious infections. It is bactericidal and active against some Gram-positive and many Gram-negative organisms. It is active against Pseudomonas aeruginosa, Proteus sp. E. coli, Klebsiella-Enterobacter-Serratia sp. Citrobacter sp. and Staphylococcus sp. Gentamicin interferes with bacterial protein synthesis by binding to 30S and 50S ribosomal subunits resulting in a defective bacterial cell membrane.

সেপটিসিমিয়া, নিওন্যাটাল সেপসিস, মস্তিস্কের আবরন-ঝিল্লির প্রদাহ এবং কেন্দ্রীয় স্নায়ুতন্ত্রের অন্যান্য সংক্রমণ, পিত্তনালীর সংক্রমণ, তীব্র পায়েলােনেফ্রইটিস অথবা প্রােস্টেট গ্রন্থির প্রদাহ, স্টেপ ভাইরিড্যানস অথবা ফিকালিস জনিত এনডােকার্ডিয়ামের প্রদাহ ইত্যাদি চিকিৎসায় যদি সংক্রমণ সমুহ জেন্টামাইসিন সংবেদনশীল জীবাণুঘটিত হয়।

Antibacterial drugsমাত্রা ও সেবনবিধি
- জেন্টামাইসিন ইঞ্জেকশন সাধারণত মাংসপেশীতে দেওয়া হয়। কিন্তু প্রয়ােজনবােধে শিরা পথে প্রয়ােগ করা যেতে পারে।
- যে সমস্ত রােগীর বৃক্ক স্বাভাবিক : দৈনিক ২-৫ মি.গ্রা. প্রতি কেজি দেহ ওজনে প্রতি ৮ ঘণ্টা অন্তর বিভক্ত মাত্রায়।
- যে সমস্ত রােগীর বৃক্কের কাজে অস্বাভাবিকতা আছে সেক্ষেত্রে উক্ত মাত্রা ১২ ঘণ্টা পর পর দিতে হবে যখন ক্রিয়াটিনিন নির্গমণের হার প্রতি মিনিটে ৫-১০ মি.লি. এর জন্য ৪৮ ঘণ্টা পর পর এবং প্রতি মিনিটে ৫ মি.লি. এর কম হলে ডায়ালাইসিসের ৩-৪ দিন পর পর প্রয়ােগ করা যেতে পারে অথবা চিকিৎসকের পরামর্শ অনুযায়ী প্রয়ােগ করা যেতে পারে।
- শিশু (২ সপ্তাহ পর্যন্ত) : প্রতি ১২ ঘণ্টায় ৩ মি.গ্রা. প্রতি কেজি দেহ ওজনে।
- ২ সপ্তাহ-১২ বৎসর : প্রতি ৮ ঘণ্টায় ২ মি.গ্রা. প্রতি কেজি দেহ ওজনে অথবা চিকিৎসকের পরামর্শ অনুযায়ী ব্যবহার্য।
The intravenous administration of Gentamicin may be particularly useful for treating patients with bacterial septicemia or those in shock. It may also be the preferred route of administration for some patients with congestive heart failure, hematologic disorders, severe burns or those with reduced muscle mass.
Administration Procedure:
- Check infusion set and infusion solution prior to use.
- Pull moderately to tear off the protective cover of the Eurohead of the bottle.
- Close the roller clamp of the giving set.
- Remove the cover of plastic lancet.
- Punch the lancet into the container.
- Hang the bottle upside down.
- Open the roller clamp to take out the solution in order to expel air.
- Close the roller clamp firmly.
- Remove the protective cover of the needle.
- Locate the venipuncture site and clean the site with an antiseptic solution.
- Puncture the vein-needle into the patient’s vein.
- Securely tape the puncture site.
- Securely tape the wings and tubing.
- Start infusion while adjusting flow rate by loosening the roller clamp.
পার্শ্ব প্রতিক্রিয়া
অন্তঃকর্ণের মধ্যপ্রদেশের ক্ষতি, বৃক্কের পরিবর্তনীয় ক্ষতি, শ্রবণেন্দ্রীয় সংক্রান্ত কর্ণের অসুবিধা হতে পারে। অন্যান্য লক্ষণগুলাে হলাে মাথা ঝিম ঝিম করা, অবসাদ, কানে ভেভোঁ শব্দ করা, কানে হট্টগােলের শব্দ হওয়া এবং কদাচিৎ শ্রবণ শক্তি লােপ পেতে পারে।
সতর্কতা
জেন্টামাইসিন এর প্রতি অতিসংবেদনশীল রােগীদের ক্ষেত্রে এটি অনুপযােগী। এটি গর্ভাবস্থায় এবং মায়াসথেনিয়া গ্র্যাভিস এর ক্ষেত্রে অনুপযােগী।
ঔষধের মিথষ্ক্রিয়া
Aminoglycosides should not be given with potentially ototoxic diuretics (e.g. frusemide and ethacrynic acid). Concurrent use of other nephrotoxic drugs including other aminoglycosides, vancomycin and some of the cephalosporins may increase the risk of toxicity. Care is also required if other drugs with a neuromuscular blocking action are given concomitantly.
গর্ভাবস্থায় ও স্তন্যদানকালে ব্যবহার
অত্যন্ত জরুরী হলেই প্রয়ােগ করার ব্যাপারে বিবেচনা করা যেতে পারে।
প্রতিলক্ষণ
এর যে কোন উপাদান এর প্রতি সংবেদনশীলতা থাকলে ব্যবহার করা যাবে না। মাঝে মাঝে ছত্রাক সহ অন্যান্য সংবেদনহীন জীবাণু দ্বারা রােগের সংক্রমণ হতে পারে। এটি ইঞ্জেকশনের জন্য নয় এবং চোখের এ্যান্টেরিয়র চেম্বারে সরাসরি প্রয়ােগ করা যাবে না।
সংরক্ষণ
Gentamicin should be stored at controlled room temperature and protected from light. This intravenous infusion is not intended for multi-dose use.
