The top 3 stings (E, B & G) keep flying out from the saddles when under tension - what am I doing wrong? The guitar has had fret-leveling and re-profiling carried out, and is in the final throes of being rebuilt.

When tightening the strings, hold the strings on the saddle until they are tight enough that they're not going to come off.
