The number one mistake is not doing the assignment or project in the first place. If you're asking for an extension, there should be extenuating circumstances. If you do the assignment, or at least put effort into doing the assignment, you show whoever you are asking that you have been trying but have faced some difficulty. Try to follow up with certain questions or specific things you had issue with when asking for an extension.
