Escitalopram is a selective inhibitor of serotonin (5-HT) re-uptake. The inhibition of 5-HT re-uptake is the only likely mechanism of action explaining the pharmacological and clinical effects of escitalopram. Escitalopram has no or low affinity for a number of receptors including 5-HT1A, 5-HT2, DA D1 and D2 receptors, α1-, α2-, β-adrenoceptors, histamine H1, muscarine cholinergic, benzodiazepine and opioid receptors.

মাত্রা ও সেবনবিধি

• প্রাপ্ত বয়স্কদের জন্য ও প্রারম্ভিক মাত্রা - সাধারনত দিনে ১০ মি.গ্রা. মাত্রা শুরু করা হয়। অন্তত এক সপ্তাহ পরে এ মাত্রা দিনে ২০ মি.গ্রা. করা যেতে পারে। এর চেয়ে বেশী মাত্রা অতিরিক্ত কার্যকরী বলে প্রমানিত হয়নি।
• বয়ােবৃদ্ধদের জন্য ৪ দিনে ১০ মি.গ্রা. একবার।
• শিশুদের ক্ষেত্রেঃ ১৮ বছরের নীচে এই ঔষধের নিরাপত্তা ও কার্যকারীতা প্রতিষ্ঠিত নয়। অথবা, চিকিৎসকের পরামর্শ অনুযায়ী ব্যবহার্য।

পার্শ্ব প্রতিক্রিয়া

অতিরিক্ত উত্তেজনা, অস্থিরতা, আছনড়ব দৃষ্টি, ডায়রিয়া, বদহজম, বমি বমি ভাব, ক্ষুধামন্দা, ক্ষুধাবৃদ্ধি, অতিরিক্ত ঘাম, সেক্সচুয়াল সমস্যা (ইচ্ছা কমে যাওয়া বা দেরিতে সফল), স্বাদ নষ্ট হওয়া, কাঁপুনি ও দৈহিক ওজনের পরিবর্তন।

সতর্কতা

During marketing of escitalopram and other SSRIs and SNRIs (Serotonin and Norepinephrine Reuptake Inhibitors), there have been spontaneous reports of adverse events occurring upon discontinuation of these drugs, particularly when abrupt, including the following: dysphoric mood, irritability, agitation, dizziness, sensory disturbances (e.g., paresthesias such as electric shock sensations), anxiety, confusion, headache, lethargy, emotional lability, insomnia, and hypomania. While these events are generally self-limiting, therehave been reports of serious discontinuation symptoms. Patients should be monitored for these symptoms when discontinuing treatment with escitalopram. A gradual reduction in the dose rather than abrupt cessation is recommended whenever possible. If intolerable symptoms occur following a decrease in the dose or upon discontinuation of treatment, then resuming the previously prescribed dose may be considered. Subsequently, the physician may continue decreasing the dose but at a more gradual rate.

ঔষধের মিথষ্ক্রিয়া

Escitalopram should not be started until 2 weeks after stopping an MAOI. Conversely, an MAOI should not be started until at least a week after escitalopram or related antidepressant has been stopped.

গর্ভাবস্থায় ও স্তন্যদানকালে ব্যবহার

গর্ভাবস্থায় ও স্তন্যদানকালে এসসিটালােগ্রামের নিরাপদ ব্যবহার প্রতিষ্ঠিত হয়নি। এ অবস্থায় ঝুঁকির চেয়ে উপকারিতা বেশি হলেই কেবল ডাক্তারের পরামর্শে এসসিটালােপ্রাম গ্রহণ করা যেতে পারে।

প্রতিলক্ষণ

এসসিটালােগ্রামের প্রতি সংবেদনশীল রােগীদের ক্ষেত্রে এটা প্রতিনির্দেশিত। মনাে এ্যামাইনাে অক্সিডেজ ইনহিবিটর গ্রহণকারীদের জন্য এটা প্রতিনির্দেশিত। ম্যানিক পর্যায়ে এসসিটালােগ্রাম স্থগিত করা উচিত। ম্যানিয়া বা খিচুনি রােগীদের ক্ষেত্রে এসসিটালােগ্রাম সতর্কতার সাথে প্রয়ােগ করা উচিত। যকৃতের সমস্যা আছে এমন রােগীদের ক্ষেত্রে সতর্কতার সাথে সর্বনিম্ন মাত্রায় (১০ মি.গ্রা./দিনে) দেয়া যেতে পারে। ডায়াবেটিসের রােগীদের ক্ষেত্রে যারা ইনসুলিন বা অন্য কোন এ্যান্টিডায়াবেটিক ওষুধ গ্রহণ করছে তাদের ক্ষেত্রেও সতর্কতার সাথে এসসিটালােপ্রাম দিতে হবে।

বিশেষ সতর্কতা

Pediatric Use: Safety and effectiveness in children below the age of 18 years have not been established.

Geriatric patients:Escitalopram pharmocokinetics in subjects age 65 and over were compared to younger subjects in a single and multi-dose study. অন্যান্য ওষুধের সাথে মিথস্ক্রিয়া

অন্যান্য কেন্দ্রীয় স্নায়ুতন্ত্রের ব্যবহারকারী ঔষধের সাথে এর ব্যবহার সাবধানতার সাথে করা উচিত। অন্যান্য মনােঅ্যামাইন অক্সিডেজ ইনহিবিটর এর সাথে ব্যবহার মারাতড়বক ঝুঁকির সৃষ্টির করে।
