I have knitted a round head for a doll - instructions now to use three needles - however the loose yarn is in the middle at the back, do I thread it through to the front?

Sometimes if it is hand sewn, then you would have to knot it. If you keep going through the front, it'll be loose. You knot it and if you still have a knot sticking out, then you can cut that piece off.
