A swollen belly is not a typical symptom of constipation, since the dried feces are usually stuck within the pelvis. Whilst some constipated cats may breath heavily while straining to go to the toilet, they don't usually when at rest. It's more likely the cat has a different problem, and should be checked over by a vet in order to identify it.
