Dentro Capsule 25 mg is a muscle relaxant drug that belongs to the hydantoin class of agents. It is used for the treatment of spasticity and to reduce muscle spasticity associated with probably irreversible conditions like spinal cord injury, cerebral palsy, multiple sclerosis etc. It is also used to treat malignant hyperthermia and is often used in conjunction with anesthetic agents.

Mechanism of Action

Dentro Capsule 25 mg works mainly by blocking the release of calcium ions (Ca2+) from the sarcoplasmic reticulum in skeletal muscles, which prevents calcium ions from binding to the troponin-C region of the muscle, and therefore the muscle remains in a relaxed state. This action interferes with excitation-contraction coupling and leads to the reduction in spasticity.

How Long Does It Take to Work?

It depends on the individual patient as to how long it takes for Dentro Capsule 25 mg to work. Generally, patients are expected to experience relief within 3 to 4 days of starting treatment with this medication.

Absorption

Dentro Capsule 25 mg is incompletely and variably absorbed from the gastrointestinal tract after oral administration, and peak concentrations are reached within 1 to 3 hours of ingestion.

Route of Elimination

The main route of elimination for Dentro Capsule 25 mg is via the biliary system via faecal elimination.

Dosage

For Use in Chronic Spasticity:
  • Adults: The following gradual titration schedule is suggested. Some patients will not respond until higher daily dosage is achieved. Each dosage level should be maintained for seven days to determine the patient's response. If no further benefit is observed at the next higher dose, dosage should be decreased to the previous lower dose.
  • Starting dose is 25 mg twice per day and it can be increased to 25-50 mg per day per week. Maximum accepted dosage is 400 mg per day.
  • Paediatric Patients: The following gradual titration schedule is suggested. Some patients will not respond until higher daily dosage is achieved. Each dosage level should be maintained for seven days to determine the patient's response. If no further benefit is observed at the next higher dose, dosage should be decreased to the previous lower dose.
  • 0.5 mg/kg once daily for seven days, then 0.5 mg/kg 3 times for 7 days, 1 mg/kg 3 times for 7 days, 2 mg/kg 3 times a day, Therapy with a dose 4 times daily may be necessary for some individuals.
For Malignant Hyperthermia
  • Preoperatively: Administer 4 to 8 mg/kg/day of oral Dantrolene in 3 or 4 divided doses for one or two days prior to surgery, with the last dose being given approximately 3 to 4 hours before scheduled surgery with a minimum of water.
  • Post Crisis Follow-up: Oral Dantrolene should also be administered following a malignant hyperthermia crisis, in doses of 4 to 8 mg/kg per day in four divided doses, for a one to three day period to prevent recurrence of the manifestations of malignant hyperthermia.

The recommended daily dosage of Dentro Capsule 25 mg for the treatment of spasticity varies with the age of the patient. It is typically started at 2.5 mg/kg/day and increased gradually to a maximum of 7.5 mg/kg/day in divided doses. The dosage should not exceed 4 g/day. Patients should be monitored for drug accumulation by regular blood tests.

Administration

Dentro Capsule 25 mg should be taken orally with meals and at bedtime with the last dose not given before sleeping. It should be taken exactly as prescribed by a physician and close medical monitoring is recommended.

Side Effect
