Aprmielast Introduction

Aprmielast is a prescription oral medication used to treat psoriatic arthritis and moderate to severe plaque psoriasis. It is an inhibitor of phosphodiesterase 4 (PDE4), an enzyme which plays a role in inflammation and immune response. Aprmielast is available in tablet form and is taken once a day.

Uses For

Aprmielast is indicated for the treatment of moderate to severe plaque psoriasis in adults and for the treatment of adults with active psoriatic arthritis. Aprmielast is not indicated for the treatment of other forms of arthritis.

Mechanism of Action

Aprmielast works by inhibiting the activity of the enzyme phosphodiesterase 4 (PDE4), which is involved inflammation. By blocking PDE4 activity, Aprmielast helps to reduce inflammation and reduce the symptoms associated with plaque psoriasis and psoriatic arthritis.

How Long Does it Take to Work?

The effect of Aprmielast can be seen within 4 to 6 weeks of treatment. Maximum effectiveness may not be seen until 12 weeks or longer. May take up to 16 weeks or longer to achieve maximum benefit.

Absorption

Aprmielast is rapidly absorbed after oral dosing. Peak concentrations are generally reached 2-3 hours after administration.

Route of Elimination

Aprmielast is eliminated from the body via renal excretion. About 65% of the absorbed dose is excreted unchanged in the urine.

Dosage

The recommended initial dosage titration of Apremilast from Day 1 to Day 5 is shown below. Following the 5-day titration, the recommended maintenance dosage is 30 mg twice daily taken orally starting on Day 6. This titration is intended to reduce the gastrointestinal symptoms associated with initial therapy. Apremilast can be administered without regard to meals.

Day 2: 10 mg in morning and 10 mg in evening
Day 3: 10 mg in morning and 20 mg in evening
Day 4: 20 mg in morning and 20 mg in evening
Day 5: 20 mg in morning and 30 mg in evening
Day 6: 30 mg twice daily

Dosage adjustment in patients with severe renal impairment. Apremilast dosage should be reduced to 30 mg once daily in patients with severe renal impairment. For initial dosage titration, it is recommended that Apremilast be titrated using only the morning schedule and the evening doses be skipped.

The recommended dosage of Aprmielast is 30 mg taken twice daily. This dosage can be increased to 60 mg twice daily if needed. Aprmielast should be taken with food in order to reduce the chance of stomach upset or diarrhea.

Administration

Aprmielast tablets should be swallowed whole with a glass of water. They should not be chewed, crushed, or broken. It is important to take Aprmielast exactly as directed by your healthcare provider.
