Introduction
Proteinuria means excess protein in the urine. Healthy kidneys filter waste while retaining most protein; damaged glomeruli or tubules leak albumin and other proteins into urine.
Transient proteinuria can follow fever, exercise, or dehydration. Persistent proteinuria suggests kidney disease—diabetic nephropathy, glomerulonephritis, hypertension-related damage, or nephrotic syndrome.
Detection is often via dipstick urinalysis or urine albumin-to-creatinine ratio (UACR). Quantifying protein loss guides diagnosis and monitors treatment response.

Overview
Microalbuminuria (30–300 mg/g creatinine) is an early marker of diabetic and vascular kidney injury. Higher levels indicate overt proteinuria and greater glomerular damage.
Treating blood pressure, glucose, and the primary renal disease slows progression to chronic kidney failure.

What happens in the body
Glomerular capillary wall injury increases permeability to albumin. Podocyte damage in diabetes and immune complex disease is a key mechanism.
Tubular proteinuria occurs when proximal tubules fail to reabsorb filtered low-molecular-weight proteins. Heavy protein loss lowers blood oncotic pressure, causing edema in nephrotic syndrome.

- Diabetes mellitus—diabetic nephropathy
- Hypertension and hypertensive nephrosclerosis
- Glomerulonephritis (IgA, lupus, membranous, FSGS)
- Nephrotic syndrome from minimal change disease or others
- Preeclampsia in pregnancy
- Fever, exercise, orthostatic proteinuria—transient benign causes
- Multiple myeloma and overflow proteinuria
Diagnosis and evaluation
Kidney evaluation confirms persistence and cause:
- Dipstick urinalysis—repeat to confirm persistence
- Urine albumin-to-creatinine ratio (preferred spot test)
- 24-hour urine protein when precise quantification needed
- Serum creatinine, eGFR, blood pressure, glucose, HbA1c
- Renal ultrasound for size and obstruction
- Serology and complement in suspected glomerulonephritis
- Kidney biopsy when diagnosis unclear or before immunosuppression
Treatment and management
Treatment addresses the underlying cause and reduces protein leak:
- ACE inhibitors or ARBs as first-line to lower intraglomerular pressure and proteinuria
- SGLT2 inhibitors in diabetic kidney disease with additional renoprotection
- Blood pressure control generally below 130/80 when tolerated
- Glycemic control in diabetes to slow nephropathy progression
- Immunosuppression for active autoimmune glomerulonephritis per specialist plan
- Diuretics for edema in nephrotic syndrome
- Dietary sodium restriction and moderate protein intake as advised
- Treat hyperlipidemia and avoid nephrotoxic drugs (NSAIDs)

Not every condition is fully preventable, but the steps below may lower risk or recurrence:
- Control diabetes and blood pressure early

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:
- Progression to chronic kidney disease and kidney failure
- Nephrotic syndrome complications: thrombosis, infection, edema
- Cardiovascular disease accelerated by chronic proteinuria
- Preeclampsia risk in pregnant women with baseline proteinuria
- Malnutrition from severe hypoalbuminemia

Living with the condition
Chronic proteinuria requires lifelong monitoring of urine albumin, blood pressure, and kidney function. Medication adherence to ACE/ARB or SGLT2 inhibitors significantly slows decline.
