Introduction

An abdominal abscess is a localized pocket of pus that forms inside the abdominal cavity because of infection or inflammation. The pus is a mix of dead tissue, bacteria, and white blood cells—the body walls off the infection instead of clearing it completely. It differs from generalized peritonitis: the infection is contained in a discrete collection rather than spreading freely throughout the peritoneum.

Abscesses commonly follow appendicitis, diverticulitis, pancreatitis, bowel perforation, surgery, or trauma. They may involve or sit next to the intestines, liver, pancreas, kidneys, or spleen. Many are polymicrobial, driven by gut organisms such as E. coli and Bacteroides species.

Typical symptoms include localized abdominal pain, fever, swelling or a tender mass, nausea, and changes in bowel habits. In children, irritability, refusal to eat, or abdominal distension may dominate; older adults may present with confusion rather than classic pain. Overview

An abdominal abscess is a localized pus collection—not scar bands (adhesions) and not dangerous rises in abdominal pressure (compartment syndrome). Pressure from the collection causes pain and organ dysfunction; systemic infection adds fever and malaise.

Most cases are acute and need prompt evaluation. Chronic abscesses are less common and often stem from incompletely treated infection. In regions with limited access to care, burdens linked to appendicitis and pyogenic liver abscess can be higher.

Contrast-enhanced CT of the abdomen/pelvis is the gold-standard diagnostic tool; ultrasound is adjunctive. Source control (drainage) plus antibiotics together form the cornerstone of treatment.

What happens in the body

  • History: symptom onset, prior surgery/infection, medications, comorbidities
  • Exam: localized tenderness, guarding/rigidity, sometimes a palpable mass
  • Bloods: CBC (elevated WBC), CRP/procalcitonin, electrolytes and renal function
  • CT abdomen/pelvis with IV contrast—main test for diagnosis, location, and drainage planning
  • Ultrasound adjunctive; MRI in selected cases; plain X-ray less specific
  • Aspiration and culture/sensitivity when needed to guide antibiotics
  • Differential: appendicitis, diverticulitis, pancreatitis, bowel obstruction—separated by imaging and clinical picture

Treatment and management

Treatment is individualized by size, location, complications, and comorbidities. Do not start or stop antibiotics on your own:

  • Start IV broad-spectrum antibiotics (e.g., piperacillin–tazobactam or a carbapenem class—clinician choice); de-escalate per culture
  • After source control, antibiotics often continue several days (about 4–7 days) per guidelines and clinical improvement
  • Abscesses larger than about 3–5 cm rarely resolve with antibiotics alone—drainage is often required
  • Ultrasound- or CT-guided percutaneous drainage—first choice in many accessible cases
  • Laparoscopic/open incision–drainage or laparotomy for complex, multiloculated, or failed drainage cases
  • Pain control, IV fluids, and nutritional support; selected observation for tiny asymptomatic collections
  • During recovery: easily digestible food, adequate fluids; stop smoking and resume walking gradually

Prevention, self-care, and lifestyle

Not every condition is fully preventable, but the steps below may lower risk or recurrence:

  • Seek care promptly for abdominal infections or suspected appendicitis—before an abscess forms
  • Safe food and water practices and hand hygiene lower enteric infection risk
  • Control diabetes, Crohn’s disease, and similar conditions to reduce recurrence
  • Reduce smoking/alcohol and maintain balanced diet and activity for overall infection resilience
  • After a prior abscess, seek follow-up quickly if symptoms return; imaging when indicated

Possible complications

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:

  • Sepsis and organ failure
  • Rupture with spread of infection throughout the abdomen
  • Fistula formation—especially with intestinal involvement
  • Recurrent abscesses when underlying disease persists
  • Damage to adjacent organs
  • Chronic pain and prolonged hospitalization
  • Dehydration and malnutrition slowing recovery

When to see a doctor or seek emergency care

Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:

  • Severe abdominal pain that is not improving
  • High fever (especially >38.9°C / 102°F) with chills
  • Persistent vomiting—unable to keep food or fluids down
