Introduction

Portal hypertension is elevated blood pressure in the portal venous system that drains the intestines, spleen, and pancreas into the liver. It most often results from cirrhosis or scarring that blocks flow through the liver sinusoids.

Complications include varices (enlarged veins in the esophagus and stomach that can bleed massively), ascites (fluid in the abdomen), and hypersplenism. Reduced liver function may coexist.

Management combines treating underlying liver disease, preventing variceal bleeding with medications and endoscopy, and controlling ascites with diuretics and sodium restriction.

Overview

Normal portal pressure is ~5–10 mmHg; clinically significant portal hypertension is generally above ~10 mmHg with risk rising as pressure increases.

Non-cirrhotic causes include portal vein thrombosis, schistosomiasis, and congenital hepatic fibrosis—important in specific regions and age groups.

What happens in the body

Increased resistance to portal flow (intrahepatic scarring or extrahepotic obstruction) plus increased portal inflow raises pressure. Collateral channels open to bypass the liver, forming varices and caput medusae.

Splanchnic vasodilation and sodium retention worsen ascites. Bacterial translocation increases infection risk.

  • Scar tissue or clot blocks portal flow

  • Hypersplenism → low platelets and white cells

Signs and symptoms

Early portal hypertension may be silent; complications bring symptoms:

  • No symptoms until complications develop
  • Vomiting blood or black tarry stools (variceal bleed)
  • Abdominal swelling from ascites
  • Enlarged spleen—fullness left upper abdomen
  • Easy bruising from low platelets
  • Fatigue and jaundice if liver synthetic function impaired
  • Confusion (hepatic encephalopathy) in advanced cirrhosis
  • Spider angiomas and palmar erythema on skin exam

Causes and risk factors

Any condition impeding portal flow can cause portal hypertension:

  • Cirrhosis from alcohol, NAFLD/NASH, hepatitis B or C
  • Portal vein thrombosis
  • Schistosomiasis (endemic areas)
  • Congenital hepatic fibrosis and rare vascular malformations
  • Right-sided heart failure raising hepatic congestion
  • Non-cirrhotic portal fibrosis (idiopathic portal hypertension)
  • Sarcoidosis and other infiltrative liver diseases

Diagnosis and evaluation

Clinical assessment plus imaging and endoscopy:

  • History of liver disease, alcohol use, or hepatitis
  • Exam: ascites, splenomegaly, stigmata of chronic liver disease
  • Labs: liver enzymes, bilirubin, albumin, INR, platelet count
  • Abdominal ultrasound with Doppler of portal vein
  • Transient elastography or liver biopsy for fibrosis stage when needed
  • Upper endoscopy to screen for esophageal/gastric varices
  • CT or MR angiography if vascular thrombosis suspected

Treatment and management

Prevent bleeding, control ascites, treat underlying liver disease:

  • Non-selective beta blockers (propranolol, carvedilol) for variceal prophylaxis
  • Endoscopic variceal band ligation for high-risk or bleeding varices
  • IV octreotide and antibiotics during acute variceal hemorrhage
  • TIPS (transjugular intrahepatic portosystemic shunt) for refractory bleeding or ascites
  • Spironolactone and furosemide for ascites; sodium restriction
  • Paracentesis for tense or symptomatic ascites
  • Treat alcohol use disorder and viral hepatitis with antivirals
  • Liver transplant evaluation for decompensated cirrhosis

Prevention, self-care, and lifestyle

Not every condition is fully preventable, but the steps below may lower risk or recurrence:

  • Prevent cirrhosis: limit alcohol, treat hepatitis, manage metabolic syndrome
  • Screen varices in known cirrhosis per guidelines
  • Low-sodium diet once ascites present
  • No NSAIDs—they worsen renal function and bleeding risk
  • Hepatitis B vaccination if not immune
  • Regular hepatology follow-up for compensated cirrhosis

Possible complications

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:

  • Acute variceal hemorrhage—high mortality without treatment
  • Spontaneous bacterial peritonitis in ascitic fluid
  • Hepatorenal syndrome
  • Hepatic encephalopathy
  • Refractory ascites requiring repeated paracentesis or TIPS
  • Progression to end-stage liver failure

When to see a doctor or seek emergency care

Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:

  • Vomiting blood, black stools, or dizziness with GI bleeding
  • Rapid abdominal swelling or weight gain from fluid
  • New confusion or sleep reversal in known liver disease
  • Yellowing eyes, swelling legs, or declining energy with liver risk factors
  • Known cirrhosis without recent endoscopic varice screening
  • Emergency for massive hematemesis or shock

Living with the condition

Strict sodium limits (often 2 g/day), daily weight monitoring, and no alcohol are pillars of ascites control. Take beta blockers and diuretics as prescribed; report dizziness or kidney changes.

Discuss advance care planning and transplant evaluation early in decompensated disease.

Frequently asked questions

Is portal hypertension reversible?

Pressure may fall if underlying cause improves (e.g., hepatitis C cure, alcohol abstinence early). Advanced cirrhosis often has persistent portal hypertension despite treatment.

Do all varices bleed?

No—many never bleed. Risk stratification guides beta blockers and band ligation for prevention.

Can I drink alcohol with portal hypertension?

Alcohol worsens liver injury and bleeding risk. Abstinence is strongly recommended in cirrhosis.
