Introduction
Ovarian cancer is a malignant growth arising most often from the fallopian tube or ovarian surface epithelium. Because early symptoms are vague—bloating, fullness, urinary urgency—it is frequently diagnosed at advanced stages.
It is less common than breast or cervical cancer but remains a leading cause of gynecologic cancer death. Risk increases with age, though it can occur in younger women, especially with inherited mutations.
Modern care combines surgery, chemotherapy, and targeted maintenance therapies guided by stage, histology, and BRCA/HRD status.

Overview
High-grade serous carcinoma accounts for most cases and is often linked to BRCA mutations or homologous recombination deficiency. Germ cell and sex cord–stromal tumors occur mainly in younger patients and have distinct treatment pathways.
Persistent symptoms lasting more than two to three weeks—especially after menopause—warrant gynecologic assessment. There is no proven screening test for average-risk women.
Completeness of surgical cytoreduction and response to platinum chemotherapy strongly influence prognosis.

What happens in the body
Accumulated DNA damage and defective repair pathways allow uncontrolled growth of epithelial or germ cells on the ovary, tube, or peritoneum. Spread commonly follows peritoneal seeding and lymphatic routes.
Endometriosis-associated clear cell and endometrioid subtypes reflect different biology. Hormonal and inflammatory factors may contribute in selected histologies.

- Unexplained fatigue or weight loss
- Abdominal mass or ascites in later stages
- Shortness of breath if fluid spreads to chest
Causes and risk factors
Most cases have no single preventable cause; known risk factors include:
- Increasing age, especially postmenopausal years
- Inherited BRCA1/2, Lynch syndrome, and related mutations
- Family history of breast, ovarian, or pancreatic cancer
- Endometriosis (clear cell and endometrioid subtypes)
- Nulliparity and more lifetime ovulatory cycles
- Obesity and possibly long-term hormone therapy in some studies
- Prior pelvic radiation (rare)
- Protective factors: oral contraceptive use, pregnancy, breastfeeding
Diagnosis and evaluation
Evaluation combines examination, imaging, markers, and tissue diagnosis:
- Pelvic examination and symptom review
- Transvaginal ultrasound to characterize adnexal masses
- CT chest/abdomen/pelvis for staging when cancer suspected
- CA-125 and sometimes HE4 for monitoring and preoperative assessment
- Germ cell markers (AFP, beta-hCG, LDH) when age/histology suggests
- Diagnostic laparoscopy or surgery with tissue sampling for confirmation
- Germline and tumor genomic testing for BRCA, HRD, and treatment planning
Treatment and management
Care is individualized by stage, fitness, and molecular profile:
- Primary debulking surgery when complete cytoreduction appears achievable
- Neoadjuvant chemotherapy plus interval debulking when upfront surgery is unsafe
- Platinum-taxane chemotherapy as systemic backbone for epithelial disease
- PARP inhibitor maintenance after response in BRCA-mutated or HRD-positive tumors
- Bevacizumab in selected advanced cases per guidelines
- Fertility-sparing surgery for select early germ cell or stromal tumors
- Palliative care for symptom control and quality of life at any stage
Prevention, self-care, and lifestyle
Not every condition is fully preventable, but the steps below may lower risk or recurrence:
- No reliable population screening—know persistent symptom red flags
- Genetic counseling and testing when family history suggests hereditary risk
- Risk-reducing salpingo-oophorectomy discussed with BRCA carriers after childbearing
- Opportunistic salpingectomy during benign gynecologic surgery may reduce risk
- Maintain healthy weight and avoid tobacco
- Report persistent bloating, pelvic pain, or urinary changes promptly
Possible complications
Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:
- Bowel obstruction, ascites, and malnutrition in advanced disease
- Treatment side effects: neuropathy, fatigue, myelosuppression
- Recurrence requiring further surgery or systemic therapy
- Fertility loss after bilateral oophorectomy
- Thromboembolism and infection during treatment
- Emotional and financial burden of chronic cancer care
When to see a doctor or seek emergency care
Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:
- Persistent bloating, pelvic pain, or urinary changes for more than 2–3 weeks
- Postmenopausal bleeding or new abdominal mass

Does the Pap smear detect ovarian cancer?
No. Pap tests screen for cervical cancer. Ovarian cancer has no proven routine screening test for average-risk women.
Should all patients have genetic testing?
Guidelines recommend germline testing for all epithelial ovarian, fallopian tube, and primary peritoneal cancers; results may guide family screening and maintenance therapy.
Can ovarian cancer be cured?
