Introduction
Liver failure means large parts of the liver are damaged beyond repair and can no longer carry out essential work: making bile, processing nutrients into energy, clearing toxins, producing clotting factors, and storing iron and vitamins. Without enough working liver tissue, the whole body is at risk.
Most liver failure develops gradually over months to years (chronic liver failure), often after cirrhosis from viral hepatitis, long-term heavy alcohol use, or other chronic liver disease. A less common form—acute liver failure—appears over days to weeks (sometimes within 48 hours) and can be hard to recognize at first.
Early clues such as nausea, poor appetite, fatigue, or diarrhea are nonspecific. Later signs—jaundice, easy bleeding, a swollen abdomen, confusion (hepatic encephalopathy), extreme sleepiness, or coma—need emergency care.

Overview
The liver is the largest internal organ and is vital for survival. When too many hepatocytes are lost, toxins build up, clotting falters, fluid may accumulate in the abdomen, and brain function can be impaired.
Chronic failure usually follows years of injury ending in cirrhosis. Acute failure has different triggers—especially acetaminophen (paracetamol) overdose, certain viruses, drug or herbal reactions, and poisonous mushrooms—and needs intensive hospital care.
Outlook depends on cause, how quickly care starts, and whether residual liver tissue can recover. Some acute cases reverse with antidotes or supportive care; advanced chronic failure often requires transplant evaluation.

What happens in the body
Widespread hepatocyte death and scarring leave too little functional tissue. Detoxification slows, ammonia and other toxins rise, bile pigments cause jaundice, and low clotting-factor production leads to bleeding tendency.
In chronic disease, portal hypertension drives ascites and varices. In acute failure, rapid necrosis can cause cerebral edema, severe coagulopathy, and multi-organ failure even before cirrhosis develops.

Signs and symptoms
Symptoms change with stage. Early features are easy to miss; advanced features need urgent care:

- Yellowing of skin or eyes (jaundice)
- Easy bruising or bleeding
- Swollen abdomen from fluid (ascites)
- Mental fog, confusion, or disorientation (hepatic encephalopathy)
- Excessive sleepiness or difficulty staying awake
- Coma in advanced failure
- Dark urine or pale stools when bile flow is impaired
- Itching of the skin
- Leg swelling from fluid retention
- Musty or sweet breath odor in some advanced cases
- Fever or shivering if infection (for example spontaneous bacterial peritonitis) develops
Causes and risk factors
Chronic and acute liver failure often have different drivers:
- Chronic hepatitis B or C infection
- Long-term heavy alcohol use
- Cirrhosis from any cause
- Hereditary hemochromatosis (excess iron storage)
- Severe malnutrition in some settings
- Acetaminophen (paracetamol) overdose—leading cause of acute failure in many regions
- Hepatitis A, B, or C and other viruses (acute presentation, including in children)
- Reactions to certain prescription or herbal medicines
- Ingestion of poisonous wild mushrooms
- Autoimmune hepatitis, acute fatty liver of pregnancy, and other less common triggers
- Metabolic dysfunction–associated steatotic liver disease progressing to cirrhosis
- Wilson disease or other inherited metabolic liver disorders (especially in younger patients)
Diagnosis and evaluation
Suspected liver failure is a hospital evaluation. Typical steps include:
- Urgent history: medicines (including acetaminophen and herbs), alcohol, viral exposures, travel, mushrooms, prior liver disease
- Exam: jaundice, ascites, mental status, bleeding signs, vital signs
- Blood tests: liver enzymes, bilirubin, INR/prothrombin time, albumin, glucose, ammonia when indicated, viral serologies, acetaminophen level
- Imaging: ultrasound or CT/MRI to assess liver size, vessels, and complications
- Arterial blood gas and kidney function—multi-organ involvement is common
- Differential: severe sepsis, drug overdose without primary liver disease, biliary obstruction, ischemic hepatitis
Treatment and management
Care is cause-specific and often intensive. Do not self-treat suspected liver failure:
- Emergency hospital/ICU care for acute liver failure
- N-acetylcysteine and supportive care when acetaminophen toxicity is confirmed or suspected
- Supportive care for viral acute failure while the liver attempts recovery
- Stop offending drugs or toxins immediately under medical guidance
- Manage encephalopathy, coagulopathy, infection, kidney injury, and fluid balance
- For chronic decompensated disease: treat complications, optimize nutrition, and control portal hypertension
- Liver transplant evaluation when recovery is unlikely or end-stage disease is established
- Treat underlying hepatitis, alcohol use disorder, or metabolic disease to protect remaining liver
- Avoid further hepatotoxins (including unsupervised acetaminophen and alcohol)
Prevention, self-care, and lifestyle
Not every condition is fully preventable, but the steps below may lower risk or recurrence:
- Get hepatitis A and B vaccination when recommended
- Eat a balanced diet and maintain a healthy weight
- Drink alcohol only in moderation—or not at all if you have liver disease; never combine alcohol with acetaminophen excess
- Wash hands after using the toilet and before handling food
- Do not share razors, toothbrushes, or needles; practice safer sex
- Ensure tattoos and piercings use sterile equipment
- Use acetaminophen only at recommended doses; check combination cold remedies
- Seek early care for chronic hepatitis, fatty liver, or heavy alcohol use before cirrhosis develops
Possible complications
Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:
- Hepatic encephalopathy progressing to coma
- Life-threatening bleeding from coagulopathy or varices
- Ascites, infection, and kidney failure (hepatorenal syndrome)
- Cerebral edema in acute liver failure
- Multi-organ failure and death without timely transplant or recovery
When to see a doctor or seek emergency care
Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:
- Jaundice with confusion, extreme sleepiness, or personality change—emergency care
- Vomiting blood, black stools, or uncontrolled bleeding
- Rapid abdominal swelling, severe pain, or high fever
- Known liver disease with sudden worsening of fatigue, bruising, or mental clarity
- Suspected acetaminophen overdose or mushroom ingestion—seek emergency care immediately
Living with the condition
If you have cirrhosis or prior decompensation, keep all hepatology appointments, follow salt and fluid guidance, and know early warning signs of encephalopathy.
Carry a medication list; ask before starting any new drug, supplement, or herbal product.
