What happens in the body

Mucosal erosion, inflammation, or tumour disrupts a blood vessel so blood enters the gut lumen. Blood that travels farther through the upper tract is digested and darkens into melena; bleeding closer to the anus exits as bright-red blood.

  • Differential: peptic ulcer, GERD-related erosions, IBD, cancer, vascular malformations

Treatment and management

Treatment depends on the underlying cause and severity; even seemingly mild cases need proper evaluation:

  • Intravenous fluids and transfusion when needed to stabilise
  • Proton pump inhibitors for peptic ulcer acid control and healing
  • Guided antibiotic course when H. pylori is present
  • Endoscopic cautery or clips to stop vessel bleeding
  • Surgery if medicines and endoscopy fail or a tumour must be removed
  • Stop smoking, limit alcohol, and manage stress
  • Fibre-rich diet to support diverticular and haemorrhoid health
  • Age- and comorbidity-adjusted plans for children and older adults
