Introduction

Esophageal varices are abnormally enlarged veins in the lower esophagus—the food pipe that carries swallowed material from the throat to the stomach. They develop most often in people with serious liver disease, especially advanced cirrhosis.

When scar tissue or a clot blocks normal blood flow through the liver, pressure rises in the portal vein (portal hypertension). Blood is forced into smaller, fragile vessels along the esophagus and stomach that were never meant to carry that volume.

Those thin-walled veins can leak or rupture. Bleeding from esophageal varices is a medical emergency that can cause vomiting of blood, black tarry stools, shock, and death if not treated promptly.

Overview

Cirrhosis from chronic hepatitis, long-term heavy alcohol use, fatty liver disease, and other liver injuries is the most common setting for esophageal varices. Not everyone with cirrhosis develops them, but risk rises as liver scarring and portal pressure worsen.

Many people have no symptoms from the varices themselves until bleeding begins. Signs of advanced liver disease—jaundice, ascites, spider nevi, palmar erythema—raise suspicion that varices may be present and need endoscopic screening.

Management focuses on lowering portal pressure, banding high-risk veins, treating acute bleeds, and addressing the underlying liver disease. In selected severe cases, shunts or liver transplant are considered.

What happens in the body

Scar tissue or thrombosis impairs flow through the liver, so pressure climbs in the portal venous system. Blood seeks alternative pathways through portosystemic collaterals, including submucosal veins of the distal esophagus.

These collaterals dilate into varices. High portal pressure, large variceal size, and red markings on the vein wall increase the chance of rupture. When a varix bleeds, blood loss can be massive and rapidly destabilizing.

Signs and symptoms

Varices themselves may cause no pain. Seek care urgently for bleeding signs, and watch for cirrhosis features that suggest screening is needed:

  • Vomiting blood (hematemesis)
  • Black, tarry stools (melena) or visible blood in stool
  • Lightheadedness, fainting, or shock after gastrointestinal bleeding
  • Jaundice (yellowing of skin or eyes) with known liver disease
  • Spider-like clusters of tiny vessels under the skin (spider nevi)
  • Reddening of the palms (palmar erythema)
  • Fluid build-up in the abdomen (ascites)
  • Enlarged spleen
  • Hand deformity consistent with Dupuytren contracture in some cirrhosis patients
  • Testicular shrinkage in some men with advanced liver disease
  • Fatigue, easy bruising, or confusion from advanced liver failure
  • No daily “ache” from the varices until rupture occurs

Causes and risk factors

Anything that raises portal pressure can contribute. Common drivers and bleed-risk amplifiers include:

  • Cirrhosis from viral hepatitis, alcohol-related liver disease, or other chronic liver injury
  • Portal vein obstruction or thrombosis
  • Severe portal hypertension
  • Large varices seen on endoscopy
  • Red marks or streaks on varices at endoscopy
  • Advanced liver failure or decompensated cirrhosis
  • Ongoing heavy alcohol use raising bleed risk
  • Not caused by ordinary heartburn alone without portal hypertension
  • Not explained by diet stress alone in the absence of liver/portal disease

Diagnosis and evaluation

Once significant liver disease is known or suspected, clinicians look specifically for varices and bleeding risk:

  • Clinical assessment for cirrhosis and portal hypertension
  • Esophagogastroduodenoscopy (EGD) to see vein size and red signs
  • CT or MRI to evaluate liver morphology and portal circulation when needed
  • Capsule endoscopy in selected patients who cannot or will not undergo standard endoscopy
  • Blood tests for liver function, clotting, and anemia after bleeding
  • Urgent endoscopy during active upper GI bleeding for diagnosis and therapy

Treatment and management

Priorities are stop active bleeding, prevent rebleeding, and treat the liver disease that drives portal pressure:

  • Beta blockers to lower portal pressure and reduce first-bleed or rebleed risk when appropriate
  • Endoscopic band ligation to tie off high-risk or bleeding varices
  • Medicines such as octreotide (or similar vasoactive agents) with endoscopy during acute bleeding
  • Blood products, airway protection, and intensive care for major hemorrhage
  • TIPS (transjugular intrahepatic portosystemic shunt) when bleeding is refractory or recurrent—balanced against risks such as encephalopathy
  • Liver transplant evaluation for severe liver disease or recurrent life-threatening bleeds
  • Antibiotics and guideline-based supportive care during variceal bleed admissions
  • Long-term alcohol cessation support and treatment of viral hepatitis or other liver causes

Prevention, self-care, and lifestyle

Not every condition is fully preventable, but the steps below may lower risk or recurrence:

  • Treat and monitor chronic liver disease with a specialist team
  • Avoid alcohol if you have cirrhosis or known varices
  • Attend scheduled screening or surveillance endoscopy when recommended
  • Take prescribed portal-pressure–lowering medicines as directed
  • Vaccinate against hepatitis A and B when indicated; avoid hepatotoxic misuse of medicines
  • Seek care early for new jaundice, swelling, black stools, or vomiting blood

Possible complications

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:

  • Massive upper gastrointestinal bleeding and hemorrhagic shock
  • Aspiration of blood and respiratory failure
  • Recurrent bleeding after an initial episode
  • Worsening hepatic encephalopathy after shunting procedures
  • Esophageal scarring or ulcers after banding (usually manageable)
  • Death if bleeding is not controlled promptly
  • Progression of underlying liver failure

When to see a doctor or seek emergency care

Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:

  • Vomiting blood or coffee-ground material
  • Black tarry stools or large amounts of rectal bleeding
  • Fainting, severe weakness, or confusion with known liver disease
  • Sudden abdominal swelling with distress
  • Known cirrhosis with new jaundice or drowsiness
  • Inability to keep fluids down after a bleed scare

Living with the condition

Living with varices means living with the liver disease behind them. Keep clinic and endoscopy appointments, know your medicines, and have a clear plan for where to go if bleeding starts.

Avoid NSAIDs and alcohol unless a clinician specifically clears them. Ask before starting herbal products that may harm the liver or affect clotting.

Family members should recognize bleed warning signs and be ready to call emergency services—do not wait overnight after vomiting blood.

Frequently asked questions

Are esophageal varices the same as hemorrhoids?

No. Both can involve dilated veins, but esophageal varices form in the food pipe from portal hypertension and can cause life-threatening bleeding. Hemorrhoids are dilated veins in the anal canal with a different risk profile.

Can I feel varices before they bleed?

Usually not. Many people have no local symptoms until bleeding. Screening endoscopy in cirrhosis is how clinicians find them early.

Does every person with cirrhosis get varices?

No, but risk rises as liver disease advances. Your clinician decides if and how often endoscopy is needed based on severity and other non-invasive markers.

Is band ligation a cure?

Banding treats the dangerous veins and lowers bleed risk, but portal hypertension and liver disease remain. Ongoing medical care and sometimes repeat banding are still needed.

What is TIPS used for?

TIPS creates a pathway inside the liver to lower portal pressure when bleeding cannot be controlled or keeps returning. It can help stop bleeding but may worsen confusion in some patients and is not first-line for everyone.

Can lifestyle changes help?

Yes for the underlying liver disease—especially stopping alcohol, treating viral hepatitis, managing weight and metabolic disease, and adhering to prescribed therapy. Lifestyle alone does not “tie off” existing high-risk varices.
