Introduction

Critical limb ischemia (CLI) is the most severe stage of peripheral artery disease, when blood flow to the leg or foot is so reduced that rest pain, non-healing ulcers, or gangrene develop. It is not ordinary “leg pain”—it is a limb- and life-threatening problem.

Atherosclerosis narrows or blocks the arteries that supply the lower limb. The same disease process often affects the heart and brain, so people with CLI also face high risks of heart attack and stroke.

Diabetes, smoking, high blood pressure, high cholesterol, chronic kidney disease, and older age are major drivers. Many people present late, after a toe or part of the foot has already turned dark or developed an open sore.

Overview

CLI represents end-stage chronic limb ischemia: ischemic rest pain (often worse at night), ischemic ulceration, or gangrene. It differs from intermittent claudication, in which pain occurs mainly with walking and eases with rest.

Prompt vascular assessment can save a limb. Ankle–brachial index (ABI), Doppler ultrasound, and angiography or MRA map the level of blockage and guide revascularization.

Without timely blood-flow restoration and wound care, risks include amputation, systemic infection, and major cardiovascular events. Smoking cessation and medical risk-factor control remain essential alongside procedures.

What happens in the body

Atherosclerotic plaque narrows or occludes lower-limb arteries, cutting oxygen and nutrient delivery. In diabetes, microvascular disease further impairs wound healing.

Chronic ischemia triggers ischemic nerve pain (rest pain) and can progress to skin and muscle necrosis—ulcers and gangrene. Infected wounds may seed systemic sepsis. Widespread atherosclerosis also raises coronary and cerebrovascular event risk.

  • Severe pain in the foot or toes at rest, often worse at night
  • Relief sometimes felt when dangling the foot (not always)
  • Ulcers or sores on toes, feet, or legs that do not heal
  • Gangrene—black or dead-looking tissue
  • Coldness of the lower leg or foot
  • Pale, bluish, or mottled skin colour change
  • Weak or absent foot pulses
  • Claudication that has progressed so pain persists at rest
  • Redness, pus, or swelling around a wound suggesting infection
  • Poor nail and skin nutrition; reduced hair on the toes or shin
  • Night pain that wakes you from sleep
  • Numbness (may overlap with diabetic neuropathy)
  • Sudden severe pain with a pale, cold limb—possible acute ischemia (emergency)
  • Fever with an infected ischemic wound—urgent care needed

Causes and risk factors

CLI is driven by severely reduced arterial flow. Clinicians commonly consider:

  • Atherosclerotic peripheral artery disease—the most common cause
  • Long-term smoking or tobacco use
  • Diabetes mellitus
  • Hypertension and dyslipidemia
  • Chronic kidney disease
  • Older age and prior cardiovascular disease
  • Less often: vasculitis, thromboangiitis obliterans, or embolism
  • Rare congenital vascular problems in children
  • Obesity and inactivity as contributing risk factors

Diagnosis and evaluation

Evaluation combines history, exam, and vascular testing:

  • History of rest pain, wounds, smoking, diabetes, and prior vascular disease
  • Pulse exam, skin colour and temperature, and wound assessment
  • Ankle–brachial index (ABI)
  • Doppler ultrasound to map flow
  • CT or catheter angiography to locate blockages
  • Magnetic resonance angiography (MRA) as a non-invasive option
  • Differentiate from diabetic neuropathy, spinal stenosis, and venous disease
  • Wound or blood cultures and imaging when infection is suspected

Treatment and management

Goals are limb salvage, pain relief, infection control, and cardiovascular risk reduction:

  • Antiplatelet therapy (for example aspirin) as directed by a clinician
  • Statins plus blood-pressure and glucose control
  • Anticoagulation only when individually indicated
  • Endovascular angioplasty and/or stenting to restore flow
  • Surgical bypass for selected complex or severe occlusions
  • Wound care, antibiotics, and debridement when needed
  • Pain control and nutritional support
  • Complete smoking cessation—one of the highest-impact lifestyle steps
  • Supervised exercise after revascularization when appropriate
  • Limited amputation when irreversible gangrene threatens life or sepsis control

Prevention, self-care, and lifestyle

Not every condition is fully preventable, but the steps below may lower risk or recurrence:

  • Stop all tobacco products and avoid secondhand smoke
  • Keep diabetes, blood pressure, and cholesterol under regular control
  • Follow a heart-healthy eating pattern and maintain a healthy weight
  • Stay physically active as advised by your clinician
  • Inspect feet daily and wear protective footwear—especially with diabetes
  • Attend scheduled vascular and cardiovascular check-ups
  • Keep vaccinations and hygiene practices up to date to reduce infection risk

Possible complications

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:

  • Limb amputation for gangrene or unsalvageable infection
  • Wound infection progressing to sepsis
  • Heart attack and stroke
  • Chronic pain and limited walking ability
  • Repeated hospital admissions
  • Graft or stent failure after revascularization
  • Decline in quality of life and mental health strain

When to see a doctor or seek emergency care

Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:

  • Severe foot or leg pain at rest
  • A wound that will not heal, or black/dead tissue
  • Redness, pus, or fever around a foot or leg wound
  • A cold, pale, or colour-changing limb with weak pulses
  • Sudden severe pain and a pale limb—seek emergency care
  • New blisters or ulcers in someone with diabetes
  • Rapidly worsening claudication—ask for vascular review

Living with the condition

With timely revascularization and risk-factor control, many people keep the limb and reduce pain. Daily foot checks, dry intact skin, and avoiding tight shoes are practical habits.

Do not cut or “burn off” calluses or necrotic tissue yourself. Smoking cessation and prescribed medicines matter as much as procedures.

Treat chest pain, sudden weakness, or stroke symptoms as emergencies—CLI marks systemic atherosclerosis, not only a leg problem.
