  • Abdominal imaging (ultrasound, X-ray, or CT) when structural disease is suspected
  • Colonoscopy and/or upper endoscopy with biopsy for IBD, microscopic colitis, cancer risk, or unexplained severe disease

Treatment and management

Therapy follows the cause. Do not rely on antidiarrheal drugs alone if infection, bloody diarrhea, or severe illness is possible—get clinical advice first:

  • Oral rehydration solutions; IV fluids if dehydration is significant
  • Treat proven bacterial or parasitic infection with appropriate antimicrobials
  • Careful, short-term use of antidiarrheals such as loperamide only when a clinician says it is safe
  • Anti-inflammatory, immunomodulating, or biologic therapy for IBD when indicated
  • Gluten-free diet for confirmed celiac disease
  • Lactose reduction, low-FODMAP trial, or other dietitian-guided plans for intolerances
  • Soluble fiber (for example psyllium) to firm stools in selected functional cases
  • Probiotics in some post-antibiotic or IBS settings as advised
  • Bile-acid binders or pancreatic enzyme replacement when those diagnoses apply
  • Surgery only for specific complications (strictures, severe IBD, tumors)—not routine first-line care
