Introduction
Carotid artery disease means the carotid arteries in the neck—which carry oxygen-rich blood to the brain—become narrowed or blocked by fatty plaque. When plaque ruptures or a clot forms, blood flow to part of the brain can drop suddenly, causing a transient ischemic attack (TIA, or “mini-stroke”) or a full ischemic stroke.
In early stages many people have no symptoms, so the condition can stay silent until a neurological event occurs. Risk rises with high blood pressure, diabetes, high cholesterol, smoking, obesity, inactivity, and a family history of early heart attack or stroke.
Warning symptoms often reflect temporary or lasting brain ischemia: one-sided face, arm, or leg weakness or numbness; slurred or lost speech; sudden vision loss in one eye; dizziness or loss of balance. Treating these as “stress” or fatigue wastes critical time.

What happens in the body
LDL cholesterol and inflammatory cells accumulate in the artery wall, forming plaque. Growing plaque narrows the lumen; an unstable surface can trigger platelet clots or shed fragments that lodge in smaller brain arteries (embolization), starving brain tissue of oxygen.
Long-standing hypertension injures the vessel wall; tobacco toxins damage endothelium and accelerate plaque. Autoimmune inflammation (for example lupus or rheumatoid arthritis) can add vascular injury, but classic atherosclerotic risks drive most cases.

Causes and risk factors
Atherosclerosis is the main cause. These factors raise risk or speed the process:
- Fatty plaque buildup in the carotid artery wall (atherosclerosis)
- Chronic high blood pressure
- Diabetes and poorly controlled blood glucose
- High LDL cholesterol or mixed dyslipidemia
- Smoking and regular tobacco use; secondhand smoke also harms vessels

Treatment and management
Goals are stroke prevention and risk control. Medicine or procedure choices depend on stenosis degree, symptoms, and overall health—do not start or stop therapy on your own:
- Antiplatelet therapy (for example aspirin or clopidogrel) as prescribed to reduce clotting
- Statin therapy to lower LDL and stabilize plaque
- Antihypertensive medicines to individualized blood-pressure targets
- Diabetes control with diet, activity, and medicines or insulin as needed
- Complete smoking cessation—one of the strongest risk reductions available
- Heart-healthy eating: vegetables, fruit, whole grains, lean protein; less saturated/trans fat
- Regular aerobic activity (~150 minutes moderate weekly when medically appropriate)

Prevention, self-care, and lifestyle
Not every condition is fully preventable, but the steps below may lower risk or recurrence:
- Stop all tobacco and avoid secondhand smoke
- Keep blood pressure, cholesterol, and blood sugar at agreed targets with regular checks
- Cut salt, fried and ultra-processed foods; increase vegetables, legumes, and fish
- Walk or exercise most days; reduce long sitting stretches
- Maintain healthy weight and sleep; use healthy stress-coping strategies
- Discuss carotid screening intervals with a clinician if you have multiple risks (often every 1–2 years)
- Stay current with indicated vaccines and infection prevention to limit inflammatory stress on vessels

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:
- Ischemic stroke with lasting paralysis, speech loss, or death
- Recurrent TIAs that foreshadow a major stroke
- Long-term vascular cognitive impairment (memory and attention decline)
- Further cardiovascular events, including heart attack, reflecting widespread atherosclerosis
- Post-stroke infection, depression, falls, and major quality-of-life loss
- Sudden neurological catastrophe from untreated high-grade stenosis

Living with the condition
After diagnosis, focus on a practical plan: daily medicines, a blood-pressure log, smoking-cessation support, and small realistic walking and diet goals. Teach family the signs of TIA and stroke so they can get you to hospital fast.
After endarterectomy or stenting, continue antiplatelet therapy as directed and keep follow-up ultrasound appointments. New dizziness or weakness is not a “wait and see” moment.
