Introduction

Appendicitis is inflammation of the appendix, usually after the lumen becomes blocked. The appendix swells, fills with infected material, and causes abdominal pain that classically starts near the navel and moves to the lower right side. It is one of the most common reasons for emergency abdominal surgery.

It can occur at any age but is especially frequent in adolescents and young adults (roughly 10–30 years), with a somewhat higher risk in males. Pain may worsen with walking, coughing, or pressing on the right lower abdomen. Fever, nausea, vomiting, and loss of appetite often accompany the pain.

Without timely care the appendix can perforate, forming an abscess or causing peritonitis (infection of the abdominal lining). Diagnosis combines history, examination, blood tests, and imaging such as ultrasound or CT. Most people need appendectomy (removal of the appendix); selected mild cases may be managed with antibiotics under close medical supervision.

Overview

Appendicitis is acute (or less often chronic/recurrent) inflammation of the appendix after obstruction and bacterial overgrowth.

It is a surgical emergency in most severe cases because delay raises the risk of rupture, abscess, and peritonitis.

Outcomes are excellent with prompt diagnosis and appendectomy; people recover fully and do not need the appendix for digestion.

What happens in the body

Obstruction of the appendix (by faecalith, lymphoid swelling, parasites, foreign body, stricture, or rarely tumour) traps mucus and raises pressure inside the lumen. Blood flow to the wall falls, tissue becomes ischaemic, and bacteria multiply.

White cells flood the area, producing inflammation, swelling, and severe pain. If pressure and infection continue, the wall necroses and perforates, spilling infected content into the abdomen or forming a walled-off abscess.

  • Lumen blockage → pressure rise → ischaemia and bacterial growth
  • Inflammation spreads to nearby peritoneum and organs
  • Perforation leads to peritonitis or appendicular abscess
  • Anatomic position of the appendix changes where pain is felt

Signs and symptoms

Symptoms vary with age and appendix position. Common patterns include:

  • Pain starting around the navel then shifting to the lower right abdomen
  • Pain worse with movement, coughing, or local pressure
  • Nausea, vomiting, and loss of appetite
  • Fever and general unwellness
  • Bloating or constipation/diarrhoea in some people
  • Urinary frequency or discomfort if the appendix lies near the bladder
  • Children: irritability, fever, vomiting, poor appetite with belly pain
  • Acute: severe symptoms over 24–48 hours
  • Chronic/recurrent: milder or fluctuating pain over days to weeks
  • Complicated: sudden worsening after temporary relief may suggest perforation

Causes and risk factors

Blockage and infection drive most cases. Contributing factors include:

  • Faecalith (hard stool mass) or other luminal obstruction
  • Lymphoid hyperplasia from viral or intestinal infection
  • Parasites (for example threadworms) in some regions
  • Rarely foreign bodies, trauma, tuberculosis, or tumours
  • Low-fibre diet and constipation as risk associations
  • Higher risk in adolescents/young adults and in males
  • Gastrointestinal infections and abdominal trauma as contributors
  • Not simply “gas” or indigestion—true appendicitis is inflammatory emergency

Diagnosis and evaluation

No single test is perfect; clinicians combine findings:

  • History of migrating pain, anorexia, fever, and symptom timing
  • Exam for right-lower tenderness, guarding, and vital-sign changes
  • Blood count for elevated white cells; CRP may rise in complicated disease
  • Urinalysis to help exclude stone or UTI (pus cells can still appear in appendicitis)
  • Pregnancy test and pelvic assessment when ectopic pregnancy or PID is possible
  • Ultrasound first-line in many children and pregnant patients
  • CT scan when atypical or ultrasound is inconclusive
  • Differential: gastroenteritis, ovarian pathology, kidney stone, diverticulitis, Meckel diverticulitis, cholecystitis, pancreatitis

Treatment and management

Most patients need prompt appendectomy plus antibiotics. Do not take strong painkillers that mask progression without medical advice, and do not delay emergency care:

  • Laparoscopic appendectomy: small incisions, camera guidance, usually faster recovery
  • Open appendectomy when needed for perforation, extensive contamination, or technical reasons
  • Antibiotics before and after surgery as directed
  • Abscess drainage with delayed appendectomy in selected complicated cases
  • Selected uncomplicated mild cases: antibiotic-first protocols only under specialist supervision
  • Pain control, fluids, and early mobilisation after surgery
  • Wound care; avoid heavy lifting until cleared
  • Return precautions for fever, wound redness, vomiting, or worsening pain

Prevention, self-care, and lifestyle

Not every condition is fully preventable, but the steps below may lower risk or recurrence:

  • No guaranteed prevention exists
  • Higher-fibre diets (fruits, vegetables, whole grains, legumes) may lower risk of faecal obstruction
  • Seek care early for migrating abdominal pain with fever or vomiting
  • Do not use home remedies alone when appendicitis is suspected
  • After surgery, gradual activity and adequate fluids support recovery (they do not prevent a first episode)

Possible complications

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:

  • Perforation of the appendix
  • Appendicular abscess
  • Peritonitis and sepsis
  • Wound infection or intra-abdominal collection after surgery
  • Longer hospital stay and recovery after complicated disease
  • In pregnancy: maternal–fetal risk if infection spreads—needs urgent coordinated care

When to see a doctor or seek emergency care

Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:

  • Severe or progressive abdominal pain, especially right lower quadrant
  • Pain with fever, vomiting, and inability to eat
  • Child with belly pain, fever, and refusal to walk or eat
  • Pregnant person with new abdominal pain—same-day urgent evaluation
  • Sudden worsening after a period of pain (possible perforation)
  • Post-appendectomy fever, wound drainage, or persistent vomiting

Living with the condition

After uncomplicated laparoscopic surgery many people go home within a day and resume light activity quickly, avoiding strenuous work until cleared (often 1–2 weeks, longer after open or complicated cases).

Support the incision when coughing or laughing, keep the wound clean and dry, and increase walking as comfort allows. Report red flags rather than waiting for a scheduled review if you feel suddenly worse.

There are usually no long-term digestive consequences of living without an appendix. Focus on balanced diet and gradual return to exercise once your surgeon agrees.

Is surgery always required?

Most definite appendicitis is treated with appendectomy. Selected mild, uncomplicated cases may be tried on antibiotics only under close medical follow-up; many still need surgery later.
