Introduction

Angina is chest discomfort or pain that occurs when the heart muscle temporarily does not get enough oxygen-rich blood. It is most often a symptom of coronary artery disease, in which atherosclerotic plaques narrow the coronary arteries, though microvascular disease or coronary spasm can also cause angina.

People typically describe pressure, squeezing, heaviness, or tightness in the chest, sometimes radiating to the arms, neck, jaw, or back. Episodes may be triggered by exertion, cold, heavy meals, or emotional stress and ease with rest or nitroglycerin in stable patterns—or appear at rest and escalate in unstable patterns that need urgent care.

Risk rises with unhealthy cholesterol levels, high blood pressure, smoking, diabetes or insulin resistance, obesity, metabolic syndrome, physical inactivity, unhealthy diet, and age (about 45+ in men and 55+ in women). Prior coronary heart disease or coronary microvascular disease strongly predisposes to angina.

Treatment combines lifestyle change, medicines (nitrates, antiplatelets, beta blockers, calcium channel blockers, statins, and others), and sometimes angioplasty with stenting or coronary bypass surgery. This page is general education; cardiology teams tailor care and decide urgency.

Overview

Angina reflects transient myocardial ischaemia. Stable angina follows a predictable exertional pattern; unstable angina is new, crescendo, or rest pain and is an acute coronary syndrome until proven otherwise; variant (Prinzmetal) angina relates to coronary spasm.

Diagnosis uses ECG, stress testing, echocardiography, blood tests, CT or invasive angiography as indicated. Ignoring progressive symptoms risks myocardial infarction and sudden death.

Goals are to relieve symptoms, prevent heart attack, and treat underlying atherosclerosis aggressively.

What happens in the body

When coronary flow cannot meet myocardial demand, ischaemic metabolites trigger pain signals. Fixed stenoses limit flow during exercise (stable angina). Plaque rupture or thrombosis can critically reduce flow at rest (unstable angina). Focal spasm can abruptly narrow an artery (variant angina). Microvascular dysfunction impairs small-vessel flow despite open epicardial arteries.

Repeated ischaemia can stun heart muscle, reduce exercise capacity, and—if prolonged—progress to infarction with permanent muscle damage.

Signs and symptoms

Not everyone feels classic crushing pain. Common and related features include:

  • Central chest pressure, squeezing, or heaviness
  • Pain radiating to left or both arms, neck, jaw, shoulder, or back
  • Shortness of breath with or without chest discomfort
  • Nausea, sweating, dizziness, or unusual fatigue
  • Exertional episodes that ease within minutes of rest (stable pattern)
  • Pain at rest, lasting longer, or rapidly worsening (unstable pattern—urgent)
  • Nocturnal or cyclical pain suggesting possible spasm in some people
  • Anginal equivalents in diabetes or older adults: isolated dyspnoea or fatigue
  • Mild cases: infrequent exertional discomfort
  • Severe cases: rest pain, pulmonary oedema, or haemodynamic instability

Causes and risk factors

Angina is a symptom; underlying contributors commonly include:

  • Coronary atherosclerotic narrowing (coronary heart disease)
  • Coronary microvascular disease
  • Coronary artery spasm (Prinzmetal/variant angina)
  • High LDL cholesterol and other lipid disorders
  • Hypertension
  • Smoking and second-hand smoke
  • Diabetes mellitus or insulin resistance
  • Overweight, obesity, and metabolic syndrome
  • Sedentary lifestyle and unhealthy diet
  • Increasing age and male sex / post-menopausal status as risk markers
  • Severe anaemia, tachyarrhythmias, or aortic stenosis increasing demand or reducing supply

Diagnosis and evaluation

Clinicians confirm ischaemia and define coronary anatomy or risk when appropriate:

  • History distinguishing stable from unstable features; cardiovascular exam
  • Resting electrocardiogram (ECG/EKG)
  • Exercise treadmill or pharmacological stress testing
  • Stress echocardiography or nuclear stress imaging
  • Blood tests including lipids, glucose/HbA1c, and cardiac enzymes when acute disease is possible
  • Chest X-ray when other chest causes need exclusion
  • Coronary CT angiography in selected intermediate-risk patients
  • Invasive coronary angiography when high risk or intervention is likely
  • Differential: reflux, musculoskeletal pain, anxiety, pulmonary embolism, aortic dissection

Treatment and management

Management targets symptoms and event prevention. Do not stop prescribed heart medicines suddenly or ignore rest pain:

  • Immediate urgent evaluation for rest, prolonged, or crescendo chest pain
  • Lifestyle: stop smoking, heart-healthy diet, weight management, stress reduction, supervised activity
  • Antiplatelet therapy such as aspirin when prescribed
  • Short- and long-acting nitrates for symptom relief and prevention
  • Beta blockers and/or calcium channel blockers to reduce ischaemic burden
  • Statins and aggressive risk-factor control for cholesterol, BP, and diabetes
  • Other agents (for example ranolazine) in selected refractory cases per cardiology advice
  • Percutaneous coronary intervention (angioplasty/stenting) when indicated
  • Coronary artery bypass grafting for suitable multi-vessel or complex disease
  • Cardiac rehabilitation after events or revascularisation

Prevention, self-care, and lifestyle

Not every condition is fully preventable, but the steps below may lower risk or recurrence:

  • Do not smoke; avoid second-hand smoke
  • Control blood pressure, cholesterol, and diabetes to target
  • Regular physical activity after clinician clearance
  • Mediterranean-style or similarly balanced diet; limit ultra-processed foods
  • Maintain healthy weight and sleep routines
  • Take prescribed preventive medicines consistently
  • Know personal warning symptoms and action plans for unstable pain

Possible complications

Delay, missed care, or unsafe self-medication can raise the chance of complications in some cases:

  • Myocardial infarction (heart attack)
  • Heart failure from cumulative muscle damage
  • Arrhythmias and sudden cardiac arrest
  • Progressive exercise intolerance and reduced quality of life
  • Anxiety related to recurrent pain
  • Procedural risks when angiography or surgery is required

Seek prompt medical advice or emergency care if any of the following apply—it is safer not to wait and see:

  • Any new, worsening, or rest chest pain—call emergency services if severe or lasting more than a few minutes
  • Chest pain with sweating, vomiting, fainting, or severe breathlessness
  • Known angina that suddenly occurs with less effort or at rest
  • Side effects or inability to take prescribed cardiac medicines
  • Before starting intense exercise if you have multiple cardiac risk factors

Living with the condition

Stable angina is often manageable with paced activity, medicine timing (including prophylactic nitrate use when advised), and risk-factor control. Keep nitroglycerin available if prescribed and know when lack of relief means emergency care.

Share an action plan with family: recognise unstable symptoms, emergency numbers, and medication lists. Cardiac rehab and smoking cessation programmes improve long-term outcomes.

Regular cardiology review adjusts therapy as disease or lifestyle changes. Untreated progressive symptoms should never be normalised as “just indigestion” without assessment.
