Concept:-

Introductory phase:

  • Also known as the orientation phase.
  • Begins when the nurse and the patient interact for the first time.
  • The nurse begins to know about the patient.
  • Tasks include establishing rapport, developing trust, (and formulation of a nursing diagnosis).
  • The major task is to develop a mutually acceptable contract.

Nurse’s tasks in the Introductory phase:

  • Establish rapport, trust, and acceptance
  • Establish communication
  • Gather data, including the client’s feelings, strengths, and weakness
  • Define client’s problems; set priorities for nursing intervention.
  • Mutually set goals.

  • Pre-interaction phase: it begins when the nurse is assigned to initiate a therapeutic relationship and included all that the nurse thinks, feels, or does immediately prior to the first interaction with the patient.
  • Working phase: most of the therapeutic work is carried out during the working phase.
  • Termination phase: it is a time to exchange feelings and memories and to evaluate mutually the patient’s progress and goal attainment.
