Introduction

Widebac IV Infusion 50 mg/vial is a broad-spectrum antibiotic belonging to the glycylcycline class, derived from tetracycline. It is used primarily to treat complicated infections caused by a variety of bacteria, including those resistant to other antibiotics. It is often used in cases where other treatments have failed or are not suitable.

Uses

Widebac IV Infusion 50 mg/vial is indicated for the treatment of:

  • Complicated skin and skin structure infections (cSSSI).
  • Complicated intra-abdominal infections (cIAI).
  • Community-acquired pneumonia (CAP).

It is typically used when other antibiotics are not effective or appropriate, particularly for infections caused by multi-drug-resistant organisms.

Mechanism of Action

Widebac IV Infusion 50 mg/vial works by inhibiting bacterial protein synthesis. It binds to the 30S ribosomal subunit of bacterial ribosomes, preventing the attachment of aminoacyl-tRNA to the mRNA-ribosome complex. This action disrupts bacterial protein synthesis, leading to the inhibition of bacterial growth and replication.

How Long Does It Take to Work?

The therapeutic effects of tigecycline can usually be observed within a few days of starting treatment, depending on the severity of the infection and the patient's response. Improvement in symptoms may be seen as early as 24 to 48 hours after initiation of therapy.

Absorption

Widebac IV Infusion 50 mg/vial is administered intravenously, and its absorption from the gastrointestinal tract is not relevant as it is not given orally. The drug is well-absorbed into the bloodstream when administered intravenously.

Route of Elimination

Widebac IV Infusion 50 mg/vial is primarily eliminated through the liver. It undergoes metabolism in the liver, and the metabolites are excreted mainly in the bile. Only a small fraction is excreted unchanged in the urine.

Dosage

The duration of therapy should be guided by the severity and site of the infection and the patient's clinical and bacteriological progress. The recommended duration of treatment with Tigecycline for complicated skin and skin structure infections or for complicated intra-abdominal infections is 5 to 14 days and for community-acquired bacterial pneumonia is 7 to 14 days. The recommended daily dose is as follows:

Adults: The recommended dosage regimen for Tigecycline is an initial dose of 100 mg, followed by 50 mg every 12 hours. Intravenous (IV) infusions of Tigecycline should be administered over approximately 30 to 60 minutes every 12 hours.

Pediatric use: Safety and effectiveness in pediatric patients below the age of 18 years have not been established.

Geriatric use: No unexpected overall differences in safety or effectiveness were observed between these subjects and younger subjects.

The recommended dosage of tigecycline is:

  • Loading dose: 100 mg IV, administered as a single dose.
  • Maintenance dose: 50 mg IV every 12 hours.

The duration of treatment typically ranges from 5 to 14 days, depending on the type and severity of the infection. Dosage adjustments may be necessary for patients with hepatic impairment.

Administration

Widebac IV Infusion 50 mg/vial is administered intravenously. It should be infused over a period of 30 to 60 minutes. The solution should be prepared and handled according to the manufacturer's instructions to ensure proper dosing and safety.

Side Effects

Common side effects of tigecycline include:

  • Nausea and vomiting.
  • Diarrhea.
  • Abdominal pain.
  • Headache.

Serious side effects may include:

  • Allergic reactions (e.g., rash, itching, swelling, severe dizziness).
  • Hepatic dysfunction.
  • Superinfection with non-susceptible organisms.

Toxicity

Widebac IV Infusion 50 mg/vial toxicity is rare but may occur. Symptoms of overdose or toxicity include severe gastrointestinal upset and signs of liver dysfunction. In case of suspected overdose, supportive measures should be taken, and medical attention should be sought.

Precautions

Precautions include:

  • Monitor for signs of liver dysfunction, especially in patients with pre-existing liver conditions.
  • Use caution in patients with renal impairment, although dose adjustments are not typically necessary.
  • Be aware of potential for allergic reactions and discontinue use if severe reactions occur.

Interaction

Widebac IV Infusion 50 mg/vial may interact with other medications, potentially affecting their efficacy or increasing the risk of side effects. It is essential to inform healthcare providers of all concurrent medications, including over-the-counter drugs and supplements.

Disease Interaction

Widebac IV Infusion 50 mg/vial should be used with caution in patients with:

  • Liver disease, as it may affect drug metabolism.
  • Severe renal impairment, although it is less relevant for tigecycline as it is mainly metabolized by the liver.

Drug Interaction

Widebac IV Infusion 50 mg/vial may interact with drugs that affect liver enzymes, such as certain antiepileptic drugs and some antifungal medications. These interactions could potentially alter the metabolism of tigecycline or other drugs. Monitoring for interactions is recommended.

Food Interactions

There are no significant food interactions with tigecycline, as it is administered intravenously. Dietary considerations are generally not a factor with intravenous medications.

Pregnancy Use

Widebac IV Infusion 50 mg/vial should be used during pregnancy only if clearly needed. There is limited data on the safety of tigecycline in pregnant women, and it is important to weigh the potential benefits against the risks to the fetus.

Lactation Use

It is not known whether tigecycline is excreted in human milk. Caution should be exercised when administering tigecycline to nursing mothers, and alternative treatments may be considered if necessary.

Acute Overdose

In case of acute overdose, symptoms may include severe gastrointestinal distress and liver dysfunction. Treatment involves supportive care, including monitoring and managing symptoms. There is no specific antidote for tigecycline overdose.

Contraindication

Widebac IV Infusion 50 mg/vial is contraindicated in individuals with a known hypersensitivity to tigecycline or any component of the formulation. It should not be used in patients with severe liver impairment without careful consideration and monitoring.

Use Direction

Administer tigecycline exactly as prescribed by a healthcare provider. Ensure proper intravenous administration technique and adherence to dosing guidelines for optimal efficacy and safety.

Storage Conditions

Widebac IV Infusion 50 mg/vial should be stored at room temperature (20°C to 25°C or 68°F to 77°F) in a tightly closed container. Protect from light and keep out of reach of children. Avoid freezing or overheating the solution.

Volume of Distribution

Widebac IV Infusion 50 mg/vial has a volume of distribution of approximately 7.5 liters/kg. This indicates extensive distribution into tissues and fluids, including the liver and other organs.

Half Life

The terminal half-life of tigecycline is approximately 27 hours. This long half-life allows for dosing every 12 hours, contributing to effective and sustained antibacterial activity.

Clearance

Widebac IV Infusion 50 mg/vial clearance is primarily hepatic, with a minimal amount being excreted unchanged in the urine. The drug is metabolized in the liver and eliminated through the bile.
