Introduction

Warfarin sodium is an anticoagulant or blood-thinning drug that is used to prevent and treat blood clots in veins and arteries. It is used to reduce risks of blood clot formation or development of a dangerous condition known as thromboembolism. It is given in the form of oral tablets.

Uses

Warin Tablet 5 mg is used for treating blood clots such as deep vein thrombosis (DVT) and pulmonary embolism (blood clots in the lungs) as well as to reduce the risk of stroke and other complications in people with atrial fibrillation not caused by a heart valve problem. Warfarin sodium may used in post-operative cases to prevent formation of blood clots. Other uses of Warin Tablet 5 mg include prevention of heart attack, heart failure, or peripheral vascular disease. Warfarin sodium may also be used for other conditions as determined by health care provider.

Mechanism of Action

Warfarin sodium blocks the action of the liver enzyme vitamin K epoxide reductase, resulting in decreased availability of functional vitamin K and diminished synthesis of vitamin K-dependent clotting factors. It also reduces the conversion of vitamin K-dependent clotting factors to their active forms. This reduces the quality and quantity of clotting factors, leading to anticoagulation.

How long does it take to work?

It usually takes 1 to 2 days for Warin Tablet 5 mg to start showing effects. However, it may take longer depending on the individual's medical condition.

Absorption

Warin Tablet 5 mg is well absorbed from the gastrointestinal tract, with an absolute bioavailability of 60 to 80%. Peak levels are reached within 1 to 4 hours of an oral dose.

Route of Elimination

Warin Tablet 5 mg is eliminated primarily by biotransformation and subsequent renal elimination of its metabolites, with an estimated elimination half-life of 32 to 38 hours.

Dosage

Whenever possible, the baseline prothrombin time should be determined but the initial dose should not be delayed whilst awaiting the result.

Use in adults: The usual adult induction dose of warfarin is 10 mg daily for 2 days. The subsequent maintenance dose depends upon the prothrombin time, reported as INR (international normalized ratio). The daily maintenance dose of warfarin is usually 3 to 9 mg (taken at the same time each day). The maintenance dose is omitted if the prothrombin time is excessively prolonged. Once the maintenance dose is established in the therapeutic range, it is rarely necessary to alter. In emergencies, anticoagulant therapy should be initiated with heparin and warfarin together. Where there is less urgency, as in patients disposed to or at special risk of thromboembolism, anticoagulant therapy may be initiated with warfarin alone. Control tests must be made at regular intervals and maintenance dosage further adjusted according to the results obtained.

Use in children: Safety and efficacy in children <18 years old have not been established. However, there is evidence of use and the initial dose is usually 0.1 mg.kg-1.d-1 adjusted subsequently to aim for an INR range the same as in adults.

The dosage should be adjusted based on the individual's required effect, medical condition, and response to treatment. Warin Tablet 5 mg tablets are available in various strengths - 2mg, 3mg, and 5mg. Most people starting on warfarin sodium require 2mg to 5mg daily, taken orally once a day.
