Introduction

Perindopril arginine is a long-acting, blunt-acting angiotensin-converting-enzyme (ACE) inhibitor with antihypertensive properties. It is used for the treatment and prevention of heart failure, hypertension (high blood pressure), and post-myocardial infarction (heart attack). Its active metabolite perindoprilat is thought to act by blocking the angiotensin-converting-enzyme, resulting in a decrease in blood pressure.

Mechanism of Action

How Long Does it Take to Work?

Perindopril arginine may take several weeks to become fully effective, and the full effect may not be seen until 6-8 weeks after starting the drug. The drug should be used as directed by your doctor for optimal results.

Absorption

Perindopril arginine is rapidly absorbed following oral administration, with peak plasma levels within 1-4 hours. The bioavailability of perindopril is low (<25%) and is decreased with food.

Route of Elimination

Perindopril arginine is metabolized primarily via the liver into its active metabolite, perindoprilat, which is then excreted renally.

Dosage

Hypertension: One Perindopril 4 tablet once daily preferably in the morning. If necessary, the dose may be increased to 8 mg after 1 month of treatment. Perindopril should be taken before food.

Stable coronary artery disease: Perindopril 4 once daily for two weeks, then increased to 8 mg once daily, depending on renal function and provided that the 4 mg dose is well tolerated. Elderly patients should receive Perindopril 2 mg once daily for one week, then Perindopril 4 once daily the next week, before increasing the dose up to 8 mg once daily, depending on renal, function. The dose should be increased only if the previous lower dose is well tolerated.

Congestive heart failure: Perindopril should be started under close medical supervision at a starting dose of 2 mg in the morning. If necessary dose may be increased to 4 mg.

Elderly patients: Start at low daily dose (4 mg or less) and titrate slowly as needed. Experience with doses exceeding 8 mg is limited.

The usual starting dose of Perindopril arginine is 2-4 mg daily, either as a single dose or divided into two doses. The recommended dose for the treatment of hypertension is 4-8 mg daily; the recommended dose for the treatment of high cholesterol is 8-32 mg daily. Higher doses may be necessary in some patients for adequate control of blood pressure.

Administration

Perindopril arginine should be taken orally with or without food. It is best to take the medication at the same time each day. If a dose is missed, take it as soon as possible and then continue taking it at the regular scheduled time.

Side Effects
