Introduction

Sodium valproate is an anticonvulsant and mood-stabilizing drug primarily used to treat epilepsy, bipolar disorder, and migraine prevention. It is commonly used under brand names like Depakote and Epilim. Sodium valproate works by increasing levels of gamma-aminobutyric acid (GABA) in the brain, helping to stabilize electrical activity in neurons and prevent seizures.

Uses

Sodium valproate is used to treat various forms of epilepsy, including generalized seizures (e.g., absence seizures, tonic-clonic seizures), focal seizures, and mixed seizure types. It is also effective in the management of bipolar disorder, particularly in stabilizing mood swings, and for the prevention of migraine headaches.

Mechanism of Action

Sodium valproate increases the availability of gamma-aminobutyric acid (GABA), an inhibitory neurotransmitter in the brain, by inhibiting the enzymes that break it down. This action stabilizes neuronal activity and prevents the abnormal electrical discharges that lead to seizures. Sodium valproate may also reduce the frequency of action potentials by blocking voltage-gated sodium channels.

How Long Does It Take to Work?

The onset of action varies depending on the condition being treated. In the case of seizure control, therapeutic effects can typically be observed within a few days to weeks of consistent dosing. In bipolar disorder, mood stabilization may take several weeks. Migraine prevention usually shows benefit after several weeks of treatment.

Absorption

Sodium valproate is rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1 to 4 hours, depending on the formulation (immediate-release or extended-release). The drug is highly bioavailable, with an average oral bioavailability of around 90%.

Route of Elimination

Sodium valproate is primarily metabolized by the liver through glucuronidation and mitochondrial beta-oxidation. Approximately 30% to 50% of the drug is excreted in the urine as glucuronide conjugates, while a smaller portion is excreted unchanged.

Dosage

General Adult Dosage:

  • Epilepsy: The typical starting dose is 600 mg daily, divided into two or three doses. The dose may be increased gradually by 200 mg every three days, depending on response and tolerability. The usual maintenance dose ranges from 1,000 mg to 2,500 mg daily.

Children and Adolescents:

  • Dosage for pediatric patients is based on body weight. The initial dose is typically 10-15 mg/kg/day, and the maintenance dose ranges from 20-30 mg/kg/day. Dosing may be adjusted based on therapeutic response and serum levels.

Administration

Sodium valproate is administered orally in the form of tablets, capsules, or syrup. It can also be given as an intravenous injection in acute situations like status epilepticus. Tablets and capsules should be swallowed whole, not crushed or chewed, and should be taken with food to minimize gastrointestinal side effects.

Side Effects

Common side effects include nausea, vomiting, dizziness, tremors, drowsiness, hair loss, weight gain, and gastrointestinal upset. More serious side effects include hepatotoxicity, pancreatitis, thrombocytopenia (reduced platelet count), and hyperammonemia (high levels of ammonia in the blood). Sodium valproate is also associated with an increased risk of birth defects in pregnant women.

Toxicity

Overdose of sodium valproate can lead to central nervous system depression, respiratory distress, hypotension, and metabolic disturbances such as hyperammonemia. In severe cases, coma and death may occur. The therapeutic range for serum valproate is typically between 50-100 mcg/mL, with toxicity usually seen at levels above 150 mcg/mL.

It should also be used cautiously in patients with a history of pancreatitis or blood disorders. Regular monitoring of liver function tests and complete blood counts is recommended during therapy.

Interaction

Sodium valproate has significant drug interactions, particularly with medications that are highly protein-bound or metabolized by the liver. It may increase the serum levels of drugs such as lamotrigine, warfarin, and zidovudine. Conversely, drugs that induce hepatic enzymes, such as carbamazepine and phenytoin, may reduce serum levels of sodium valproate.

Disease Interaction

Sodium valproate should be used cautiously in patients with liver disease, urea cycle disorders, mitochondrial disease, and pancreatitis, as it may exacerbate these conditions. Patients with psychiatric disorders should be monitored closely for mood changes or worsening of depressive symptoms.

Drug Interaction

Major drug interactions include:

  • Lamotrigine: Sodium valproate can increase lamotrigine levels, leading to an increased risk of severe skin reactions like Stevens-Johnson syndrome.
  • Carbamazepine and Phenytoin: These drugs may lower sodium valproate levels by inducing hepatic enzymes, reducing its effectiveness.
  • Warfarin: Sodium valproate may increase the anticoagulant effect of warfarin, raising the risk of bleeding.

Food Interactions

Pregnancy Use

Acute Overdose

Symptoms of acute overdose include somnolence, heart block, respiratory depression, hypotension, and metabolic acidosis. In severe cases, coma and death may occur. Treatment involves supportive care, including gastric lavage, activated charcoal, and correction of metabolic imbalances. Hemodialysis may be used in cases of severe toxicity.

Contraindication

Sodium valproate is contraindicated in patients with liver disease, urea cycle disorders, and hypersensitivity to the drug. It is also contraindicated in pregnant women unless no alternative treatments are available and the potential benefits outweigh the significant risks to the fetus.

Use Direction

Sodium valproate should be taken exactly as prescribed by a healthcare provider. The drug can be taken with or without food, but consistency with meals is recommended to ensure stable absorption. Doses should be taken at the same time each day, and missed doses should be taken as soon as remembered unless it is almost time for the next dose.

Storage Conditions
