Introduction

Chlorhexidine hydrochloride + Cetrimide is a topical antiseptic combination comprising of two active ingredients chlorhexidine hydrochloride and cetrimide. It can be used to clean and disinfect skin, cuts and wounds prior to a medical procedure.

Uses

Chlorhexidine hydrochloride + Cetrimide is primarily used to reduce bacterial presence on the skin and as an anitseptic to reduce the risk of cross-infection. The combination is also used to disinfect skin before minor surgical procedures such as minor excisions, pap smears, and removal of sutures, or to treat skin and wound infections.

Mechanism of action

Chlorhexidine hydrochloride is an antimicrobial agent with wide antibacterial activity against Gram-positive and Gram-negative bacteria. It works by disrupting the integrity of the cell walls of bacteria. Cetrimide is a quaternary ammonium compound with antiseptic effects. It acts by disrupting cell membrane and denaturing proteins.

How Long Does It Take to Work?

Chlorhexidine hydrochloride + Cetrimide usually takes a few seconds or minutes to work. In some cases it may take up to 10 minutes for the cleaning and disinfecting effects to become visible.

Absorption

Chlorhexidine hydrochloride + Cetrimide is not expected to be absorbed through the skin in significant amounts.

Route of Elimination

Chlorhexidine hydrochloride + Cetrimide is not expected to be absorbed by the body, and is thus not expected to be systemically available. It is most likely eliminated through the gastrointestinal tract.

Dosage

Adult and children: Apply 2-3 times daily or as directed.

Chlorhexidine hydrochloride + Cetrimide is available in 0.5% and 1% solutions. It is applied to the affected area with a cotton pad, swab or spray. For adults the recommended solution strength is 1% and it should be applied in a circular motion. For children, the advised strength is 0.5%. The area should be allowed to air dry before applying a sterile dressing. For cleaning and disinfecting wounds, a 1% solution is recommended three to four times a day.

Chlorhexidine hydrochloride + Cetrimide should be applied externally on the affected area. It can be applied onto the skin with a cotton pad, swab or sprayed on the affected area. After the solution has been applied, it should be allowed to air dry before applying a sterile dressing.

Side Effects

The most common side effect of using Chlorhexidine hydrochloride + Cetrimide is skin irritation, itchiness, redness, and burning sensation. Some people may also experience photosensitivity.

Toxicity

Chlorhexidine hydrochloride + Cetrimide is toxic if taken orally and skin contact should be minimized and a facemask or protective clothing should be worn during application. Prolonged exposure to aerosolised preparations may cause sensitisation.

Precaution

  • Use with caution in people with a known sensitivity or allergy to either chlorhexidine hydrochloride or cetrimide.
  • Avoid contact with the eyes and mucous membranes.
  • Do not use on open wounds, as it may cause prolonged stinging or irritation.
  • Wash hands thoroughly after application of Chlorhexidine hydrochloride + Cetrimide.

Acute Overdose

An acute overdose of Chlorhexidine hydrochloride + Cetrimide is unlikely to cause serious harm. Symptoms of an overdose may include nausea, vomiting, diarrhoea, drowsiness, and dizziness.

Contraindication

Chlorhexidine hydrochloride + Cetrimide should not be used in people with known hypersensitivity or allergy to either of its active ingredients or to any of the excipients present in the formulation. It should also not be used on open wounds.

Use Direction

  • Wash and clean the affected area thoroughly before use.
  • Take a sufficient amount of Chlorhexidine hydrochloride + Cetrimide solution on a cotton pad or swab, and apply onto the affected skin area.
  • Gently rub the solution over the affected area.
  • Allow the area to air dry before application of a sterile dressing.

Storage Condition

Chlorhexidine hydrochloride + Cetrimide should be stored in a cool and dry environment, away from direct light and heat. It should be kept out of the reach of children.

Chlorhexidine hydrochloride + Cetrimide is not expected to be systemically available and hence does not have any distribution volume.
