Introduction
Norethindrone Acetate and Ethinyl Estradiol Tablets, USP, combined with Ferrous Fumarate Tablets, USP, are oral contraceptives used to prevent pregnancy. This combination product provides both estrogen (Ethinyl Estradiol) and progestin (Norethindrone Acetate), along with iron supplements (Ferrous Fumarate) for reducing anemia risk during menstruation.
Uses
- Prevention of pregnancy.
- Regulation of menstrual cycles.
- Treatment of hormonal imbalances.
Mechanism of Action
The contraceptive effect of Norethindrone Acetate and Ethinyl Estradiol is achieved through inhibition of ovulation by suppressing the gonadotropins. Ethinyl Estradiol stabilizes the endometrial lining and helps to regulate the menstrual cycle, while Norethindrone Acetate alters cervical mucus, making it difficult for sperm to enter the uterus. Ferrous Fumarate, a form of iron supplement, provides additional iron during the non-active pill days.
How Long Does It Take to Work?
When taken as directed, the contraceptive effect starts within the first 7 days of consistent use. For immediate contraceptive protection, it is recommended to start on the first day of the menstrual cycle.
Absorption
Both Norethindrone Acetate and Ethinyl Estradiol are rapidly absorbed from the gastrointestinal tract, reaching peak plasma concentrations within a few hours. Food may slightly delay absorption but does not significantly alter efficacy.
Route of Elimination
The metabolites of Norethindrone and Ethinyl Estradiol are primarily excreted in urine and feces. A significant fraction undergoes enterohepatic circulation, which contributes to the duration of drug action.
Dosage
Typically, one tablet containing Norethindrone Acetate and Ethinyl Estradiol is taken daily for 21 days, followed by 7 days of Ferrous Fumarate tablets. Dosage may vary based on the prescribing physician’s assessment and patient-specific needs.
Administration
- Oral administration at the same time daily for optimal effectiveness.
- Follow physician guidance for starting and switching between different contraceptives.
Side Effects
- Nausea and vomiting.
- Breast tenderness.
- Headache.
- Weight changes.
- Changes in menstrual flow.
Toxicity
Overdose of Norethindrone Acetate and Ethinyl Estradiol may lead to symptoms such as nausea, vomiting, and withdrawal bleeding. Severe toxicity is rare but requires medical attention if symptoms persist.
Precaution
- Assess for a history of thromboembolic disorders before prescribing.
- Monitor blood pressure and cardiovascular risk factors.
- Do not smoke while using oral contraceptives due to increased cardiovascular risk.
Interactions
Concomitant use with certain medications may reduce contraceptive efficacy or increase side effects.
Drug Interactions
- Antibiotics like rifampin may decrease contraceptive effectiveness.
- St. John's Wort may reduce serum concentrations of contraceptive hormones.
Food Interactions
Food does not significantly impact absorption but may delay peak plasma levels.
Pregnancy Use
Not recommended for use during pregnancy. Discontinue immediately if pregnancy is confirmed.
Lactation Use
Progestin-only contraceptives are generally preferred during lactation. This combination should be used with caution due to potential effects on milk production and infant health.
Acute Overdose
Symptoms may include nausea, vomiting, and vaginal bleeding. No specific antidote; treatment is symptomatic.
Contraindication
- History of thromboembolic disorders.
- Active liver disease.
- Known or suspected breast cancer.
- Pregnancy.
Use Direction
Take one tablet daily at the same time each day. Follow prescribed sequence of active hormone tablets followed by Ferrous Fumarate tablets.
Storage Condition
Store in a cool, dry place away from direct sunlight and moisture. Keep out of reach of children.
Volume of Distribution
Norethindrone and Ethinyl Estradiol have a wide volume of distribution, with high plasma protein binding, mainly to albumin and sex hormone-binding globulin (SHBG).
Half-Life
The elimination half-life for Ethinyl Estradiol is approximately 24 hours, while Norethindrone has a half-life of around 6-8 hours.
Clearance
Clearance occurs primarily through hepatic metabolism, followed by renal and fecal excretion of metabolites.
