Introduction

Eto Tablet 60 mg is a new COX-2 selective inhibitor. Current therapeutic indications are: treatment of rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, chronic low back pain, acute pain and gout. Like any other COX-2 selective inhibitor, Eto Tablet 60 mg selectively inhibits isoform 2 of cyclo-oxigenase enzyme (COX-2) to reduce the generation of prostaglandins (PGs) from arachidonic acid. It is approved in more than 60 countries worldwide but not in the US.

Uses

Eto Tablet 60 mg is a selective COX-2 inhibitor used to relieve moderate post-surgical dental pain as a short-term treatment and inflammatory and painful symptoms of various forms of arthritis.

For the treatment of rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, chronic low back pain, acute pain and gout.

Pharmacodynamics

Eto Tablet 60 mg is a COX-2 selective inhibitor (approximately 106 times more selective for COX-2 inhibition over COX-1).

Mechanism of Action

Like any other COX-2 selective inhibitor Eto Tablet 60 mg selectively inhibits isoform 2 of cyclo-oxigenase enzyme (COX-2), preventing production of prostaglandins (PGs) from arachidonic acid.

Toxicity

This reduced activity is the cause of reduced gastrointestinal toxicity, as demonstrated in several large clinical trials performed with different COXIB (see below links on NEJM and The Lancet). Some clinical trials and meta-analysis showed that treatment with COXIB lead to increased incidence of cardiovascular adverse events compared to placebo

Food Interactions

  • Take with or without food. The absorption is unaffected by food.

Dosage

Adult and adolescent over 16 years:
  • Osteoarthritis: The recommended dose is 30 mg once daily. In some patients with insufficient relief from symptoms, an increased dose of 60 mg once daily may increase efficacy.
  • Rheumatoid arthritis: The recommended dose is 90 mg once daily.
  • Ankylosing spondylitis: The recommended dose is 90 mg once daily.
  • Acute gouty arthritis: The recommended dose is 120 mg once daily. In clinical trials for acute gouty arthritis, Eto Tablet 60 mg was given for 8 days.
  • Postoperative dental surgery pain: The recommended dose is 90 mg once daily, limited to a maximum of 3 days.
Some patients may require additional postoperative analgesia. As the cardiovascular risks of Eto Tablet 60 mg may increase with dose and duration of exposure, the shortest duration possible and the lowest effective daily dose should be used. The patient's need for symptomatic relief and response to therapy should be re-evaluated periodically, especially in patients with osteoarthritis.
