Clopidogrel irreversibly inhibits the P2Y12 adenosine diphosphate (ADP) receptor on platelets, preventing the binding of ADP to its platelet receptor. This inhibition disrupts the activation of the GPIIb/IIIa receptor complex, thus inhibiting platelet aggregation. The effect persists for the lifespan of the platelet (7-10 days).
Clopidogrel starts to work within 2 hours after oral administration, but the maximum inhibition of platelet aggregation is reached after 3 to 5 days of consistent dosing. Its antiplatelet effect lasts for the life of the platelet.
Clopidogrel is rapidly absorbed after oral administration, with bioavailability of about 50%. Peak plasma concentrations are reached approximately 1 hour after dosing. However, it is a prodrug that requires hepatic activation to exert its effects.
