Mechanism of Action

Carbotin IV Infusion 10 mg/ml is an analogue of cisplatin, both of which are platinum-based chemotherapy drugs. It works by crosslinking DNA strands, leading to apoptosis in cells, and ultimately, cell death. This leads to reduced growth and division of cancer cells, which in turn leads to a reduction in the size of tumours.

How Long Does it Take to Work?

Carbotin IV Infusion 10 mg/ml usually takes 3-4 weeks to show signs of significant effectiveness. Patients may begin to experience relief from symptoms associated with their cancer within weeks of the initial treatment, although this varies from case to case.

Absorption

Carbotin IV Infusion 10 mg/ml is absorbed rapidly and almost completely after intravenous administration, with peak plasma concentrations occurring in approximately 1 – 2 hours.

Route of Elimination

Carbotin IV Infusion 10 mg/ml is excreted mainly via the kidneys. Approximately 70% is excreted in the urine as unchanged, parent drug and its metabolites, while the remainder is excreted in the feces via biliary elimination.

Dosage

Needles or intravenous sets containing aluminum parts that may come in contact with Carboplatin injection should not be used for the preparation or administration. Aluminum reacts with Carboplatin causing precipitate formation and/or loss of potency. Procedures for proper handling and disposal of anti-cancer drugs should be implemented. Several guidelines on this subject have been published. There is no general agreement that all of the procedures recommended in the guidelines are necessary or appropriate.

After dilution, Carboplatin should be used by the intravenous route only. The recommended dosage of Carboplatin in previously untreated adult patients with normal kidney function is 400 mg/m2 as a single I.V. dose administered by a 15 to 60 minute infusion. Therapy should not be repeated until four weeks after the previous Carboplatin course and/or until the neutrophil count is at least 2000 cells/mm3 and the platelet count is at least 100,000 cells/mm3 . Reduction of the initial dosage by 20-25% (i.e, 300-320 mg/m2) is recommended for those patients who present with risk factors such as prior myelosuppressive treatment and low performance status (ECOG-Zubrod 2-4 or Karnofsky below 80). For patients age 65 and over, dosage adjustment, initially or subsequently, may be necessary, dependent on the physical condition of
the patient.

Determination of the hematologic nadir by weekly blood count during the initial courses of treatment with Carboplatin is recommended for dosage adjustment for subsequent courses of therapy. The dose of Carbotin IV Infusion 10 mg/ml for adult is generally between 400-600 mg/m2 intravenously over 1-2 hours. Dosage depends on the patient’s body surface area, type of cancer and other medical conditions. It is typically given every 4 weeks.

Carbotin IV Infusion 10 mg/ml is typically given intravenously (IV) via a pump. It can also be given via intramuscular injections, although this is less common.
