Baricit Tablet 4 mg is a once-daily oral JAK (Janus-associated kinase) inhibitor approved for the treatment of adult patients with moderately to severely active rheumatoid arthritis (RA). It is available as a film-coated tablet for oral administration and sold under the brand name Olumiant. Baricit Tablet 4 mg was developed by Eli Lilly and Incyte Corporation and is currently available in the United States, European Union, Japan, and other countries.

Baricit Tablet 4 mg is used to treat adult patients with moderately to severely active rheumatoid arthritis (RA), a type of arthritis which results in inflammation and pain in the joints. Baricit Tablet 4 mg can be used as monotherapy or in combination with methotrexate or other disease-modifying anti-rheumatic drugs (DMARDs).

Mechanism of Action

Baricit Tablet 4 mg works by targeting JAK1 and JAK2, proteins which are part of the JAK-STAT signaling pathway. This pathway is important in the regulation of several processes, including inflammation. By inhibiting JAK1 and JAK2, Baricit Tablet 4 mg reduces the production of many inflammatory mediators and leads to a decrease in inflammation and pain associated with RA.

How Long Does It Take to Work?

The effects of Baricit Tablet 4 mg can be seen within 4 to 6 weeks of starting treatment. However it can take up to 4 months before the full therapeutic effects of Baricit Tablet 4 mg can be appreciated.

Absorption

Baricit Tablet 4 mg is quickly and completely absorbed from the gut following oral administration with peak plasma concentrations reached in 1 to 3 hours.

Route of Elimination

Baricit Tablet 4 mg is mainly eliminated through metabolism via CYP enzymes in the liver. The metabolites are then mainly eliminated through the faeces and to a lesser extent in the urine.

Dosage

Adult dose: The recommended dose of Baricitinib is 2 mg once daily. It may be used as monotherapy or in combination with Methotrexate or other Disease-modifying antirheumatic drugs (DMARDS). Baricitinib can be given orally with or without food.

Pediatric Use: The safety and effectiveness of Baricitinib in pediatric patients have not been established.

Geriatric Use: Because elderly patients are more likely to have decreased renal function, care should be taken in dose selection.

Hepatic Impairment: No dose adjustment is necessary in patients with mild or moderate hepatic impairment.

Renal impairment: Baricitinib is not recommended for use in patients with estimated GFR of less than 60 mL/min/I .73 m2.

The recommended starting dose of Baricit Tablet 4 mg is 4 mg administered once daily. This dose can be reduced to 2 mg for patients who experience side-effects or to increase safety in certain populations.

Administration

Baricit Tablet 4 mg should be taken orally once a day with or without food. It should be administered in the morning or evening with the goal of evening administration in order to maintain a steady state of drug concentration in the body.

Side Effects

Food Interactions

It is recommended that Baricit Tablet 4 mg be taken either with food or without food. High-fat meals may reduce the absorption of Baricit Tablet 4 mg, while a high-fiber meal may increase it.

Pregnancy Use
